Swelling and pain in the left ankle?

Hello doctor, my left ankle was sprained many years ago, and I later treated it with acupuncture.
However, two years ago, I suddenly experienced pain in my ankle while sleeping at midnight.
I hadn't twisted my ankle, but the pain increased and it became swollen.
I thought it might be similar to the previous sprain, so I treated it as such for over a week, and it eventually got better.
However, a few days ago, the same situation occurred again; my left ankle started hurting in the middle of the night, to the point where I couldn't walk.
What could be the cause of this? Please help me.
Thank you!

Hsu, 20~29 year old female. Ask Date: 2000/11/15

Chronic left ankle pain can be a complex issue, especially considering your history of an ankle sprain and the recurrent nature of your symptoms. Based on your description, it seems that your ankle pain may be related to several potential causes, which I will outline below, along with possible solutions and recommendations.

Possible Causes of Chronic Left Ankle Pain
1. Residual Effects of Previous Injury: Since you mentioned that you had a significant ankle sprain years ago, it is possible that you are experiencing residual effects from that injury. Ankle sprains can lead to chronic instability, which may cause pain during certain activities or even during rest. Scar tissue formation and changes in the joint mechanics can also contribute to ongoing discomfort.

2. Tendinitis: Inflammation of the tendons around the ankle, such as the Achilles tendon or peroneal tendons, can cause pain, especially if there has been a history of overuse or strain. Tendinitis often presents with pain that worsens with activity and improves with rest.

3. Arthritis: Osteoarthritis or post-traumatic arthritis can develop after an injury, leading to chronic pain and swelling. This condition is characterized by joint stiffness, especially after periods of inactivity, and may worsen with certain movements.

4. Nerve Issues: Conditions such as tarsal tunnel syndrome, where the tibial nerve is compressed, can lead to pain, tingling, or numbness in the ankle and foot. This might explain sudden pain episodes that occur at night.

5. Gout or Other Inflammatory Conditions: Gout is a type of arthritis caused by the accumulation of uric acid crystals in the joints, leading to sudden and severe pain, often at night. Other inflammatory conditions could also be a factor.

Recommendations for Management
1. Rest and Ice: If you experience acute pain, resting the ankle and applying ice can help reduce swelling and alleviate pain. Avoid putting weight on the affected ankle until the pain subsides.

2. Physical Therapy: Engaging in physical therapy can be beneficial for strengthening the muscles around the ankle, improving stability, and enhancing range of motion. A physical therapist can provide tailored exercises and modalities to help manage your pain.

3. Medication: Over-the-counter nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen or naproxen can help reduce pain and inflammation. However, consult with a healthcare provider before starting any medication, especially if you have other health conditions.

4. Orthotics: Custom orthotics or supportive footwear can help provide stability and alleviate pressure on the ankle joint, particularly if there is a history of instability.

5. Consultation with a Specialist: Given the recurrent nature of your symptoms, it may be wise to consult with an orthopedic specialist or a podiatrist. They can perform a thorough examination, possibly including imaging studies like X-rays or MRIs, to assess the underlying structures of your ankle and determine the best course of action.

6. Lifestyle Modifications: Consider modifying activities that may exacerbate your ankle pain. If you are involved in sports or activities that put stress on the ankle, it may be necessary to adjust your routine or incorporate low-impact exercises.

Conclusion
Chronic ankle pain can stem from various causes, particularly following an injury. It is essential to address the issue comprehensively, considering both the physical aspects and any potential underlying conditions. Seeking professional medical advice will provide you with a clearer diagnosis and a tailored treatment plan to manage your symptoms effectively. Remember, early intervention can often prevent further complications and improve your quality of life.
