Uninvited guest during menstruation?
I experienced irregular menstruation after giving birth (my second child is now 1 year and 10 months old), with my period being overdue and recently delayed for 4 months.
Concerned that my period might return like before, I took contraceptive pills on the fifth day of my period to regulate it.
However, on the second day of my period, I experienced painful uterine contractions, and after using the restroom, I expelled a soft, grayish-white tissue about 4 cm long and 2 cm wide, which had some blood.
This situation had occurred before, prior to the birth of my second child, but at that time, it was a small spherical shape expelled under similar painful contractions.
I would like to ask the doctor what this could possibly be.
Should I seek medical attention immediately? I would greatly appreciate your clarification.
Thank you!

Irregular menstrual cycles and unusual discharge after childbirth can be concerning for many women, especially when they experience significant changes in their menstrual patterns or notice unusual substances being expelled from the body. In your case, it appears that you have been experiencing irregular periods since the birth of your second child, and you have also noted the passage of a soft, grayish-white substance with blood during a painful uterine contraction.
Firstly, it is important to understand that after childbirth, a woman's body undergoes numerous hormonal changes as it returns to its pre-pregnancy state. These changes can lead to irregular menstrual cycles, especially in the first year or two postpartum. The irregularity can manifest as delayed periods, missed cycles, or changes in flow and duration. It is not uncommon for women to experience cycles that are longer or shorter than their typical pattern during this time.
The use of hormonal contraceptives, such as birth control pills, can also influence menstrual cycles. While these medications are often prescribed to help regulate periods, they can sometimes lead to unexpected side effects, including breakthrough bleeding or changes in the nature of menstrual flow.
Regarding the discharge you described, the expulsion of a grayish-white substance with blood could be indicative of several possibilities. It may represent retained products of conception, especially if it occurred shortly after a period of heavy bleeding or cramping. Retained products can occur after childbirth or miscarriage and may require medical evaluation to ensure that the uterus is clear and to prevent complications such as infection or heavy bleeding.
Another possibility is that the substance could be a result of normal uterine shedding, especially if you have recently resumed menstruation. However, the presence of significant pain and the unusual characteristics of the discharge warrant further investigation.
Given your symptoms, it is advisable to seek medical attention promptly. A healthcare provider can perform a thorough evaluation, which may include a pelvic examination, ultrasound, or blood tests to assess hormone levels and rule out any complications. It is crucial to address any potential issues early to ensure your health and well-being.
In summary, while irregular periods and unusual discharge can be common postpartum, the specific symptoms you are experiencing—especially the painful contractions and the passage of tissue—should be evaluated by a healthcare professional. It is always better to err on the side of caution when it comes to your health, and a medical evaluation can provide you with peace of mind and appropriate care.
