Erectile dysfunction and tear injuries below the glans?

Hello, doctor.
I have been troubled by two issues for about 2 to 3 years.
The first is that sometimes I cannot achieve an erection, and at times, I lose my erection halfway.
Other times, I can maintain an erection for 1 to 2 hours.
I am unsure where to start with the examinations.
Previously, when I visited Cathay Hospital and mentioned my issue, they just prescribed Viagra for me.
I am looking for ways to get checked and treated properly, rather than just taking medication.
What examinations should I undergo? I am only 33 years old, and I feel this is quite serious.

The second issue is that I often have tears or cuts at the junction of the glans and the foreskin.
Is this due to phimosis? I frequently get injured, which has reduced my sexual activity because the wounds are painful.
For the past two days, I have been searching online for treatment options and happened to find this place, which is great.
Thank you, doctor, for any advice you can provide.

Mr. Lin, 30~39 year old female. Ask Date: 2006/10/02

Dr. Lin Hexing reply Urology

Hello Mr.
Lin: Erectile dysfunction refers to the difficulty in achieving sufficient rigidity for an erection or the inability to achieve an erection at all, which prevents penetration and the ability to maintain satisfactory sexual activity for both partners.
Generally, if rigidity is present but ejaculation occurs within 15 seconds of vaginal penetration, it is classified as premature ejaculation, which is a different condition.
However, as you mentioned, you can sometimes engage in normal sexual intercourse for 1 to 2 hours, which should not be categorized as erectile dysfunction.
It is possible that the issue may be related to phimosis, which could lead to discomfort during sexual activity.
This discomfort or pain during the process of intercourse or during an erection could psychologically inhibit the erectile mechanism, resulting in inconsistent erectile performance.
You should carefully examine how much excess foreskin is present when fully erect; if there is too much excess or if it is too loose, you may want to consider circumcision as a treatment option.

Erectile dysfunction (ED) and frenulum tears are common concerns among young men, and it's important to address these issues with a comprehensive understanding of their potential causes and treatment options.

Understanding Erectile Dysfunction
1. Erectile Dysfunction in Young Men: It is indeed possible for young men, even those in their 20s and 30s, to experience erectile dysfunction. While it is often assumed that ED is primarily a concern for older men, psychological factors, lifestyle choices, and underlying health conditions can contribute to ED at any age. Stress, anxiety, depression, and relationship issues are common psychological factors that can affect sexual performance. Additionally, lifestyle factors such as poor diet, lack of exercise, smoking, and excessive alcohol consumption can also play a significant role.

2. Physical Examination and Tests: To diagnose the underlying cause of ED, a thorough medical history and physical examination are essential. Your healthcare provider may recommend several tests, including:
- Hormonal Testing: Checking testosterone levels can help determine if hormonal imbalances are contributing to your symptoms.

- Blood Tests: These can assess overall health and identify conditions like diabetes or high cholesterol that may affect erectile function.

- Penile Doppler Ultrasound: This test evaluates blood flow to the penis and can help identify vascular issues.

- Nocturnal Penile Tumescence Test: This test monitors erections during sleep to determine if the ED is psychological or physical.

3. Treatment Options: While medications like Viagra (sildenafil) are commonly prescribed, they are not the only solution. Lifestyle changes, such as improving diet, increasing physical activity, and managing stress, can significantly improve erectile function. Counseling or therapy may also be beneficial, especially if psychological factors are at play.

Understanding Frenulum Tears
1. Frenulum Tears: The frenulum is the sensitive band of tissue on the underside of the penis that connects the glans (tip) to the shaft. Frequent tearing or injury in this area can be painful and may lead to a decrease in sexual activity due to discomfort.
2. Causes of Frenulum Tears:
- Short Frenulum: A short frenulum can lead to tension during erections, causing it to tear more easily.

- Friction: Excessive friction during sexual activity or masturbation can also contribute to tearing.

- Hygiene and Skin Conditions: Poor hygiene or skin conditions can exacerbate sensitivity and lead to injuries.

3. Management and Treatment:
- Consultation with a Urologist: A urologist can assess whether you have a short frenulum or other anatomical issues. They may recommend a frenuloplasty, a minor surgical procedure to lengthen the frenulum and reduce the risk of tearing.

- Gentle Techniques: Using lubrication during sexual activity can help minimize friction and reduce the risk of tearing.

- Avoiding Irritants: Be cautious with soaps or products that may irritate the sensitive skin of the penis.

Conclusion
Both erectile dysfunction and frenulum tears can significantly impact a young man's quality of life and sexual health. It is crucial to seek professional medical advice to explore the underlying causes and appropriate treatment options. Open communication with your healthcare provider about your symptoms and concerns is vital for effective management. Remember, you are not alone in facing these issues, and there are many resources and treatments available to help you regain confidence and improve your sexual health.
