What are the white spots on the lower eyelid?

Dear Doctor: I used to frequently have eye discharge, and my previous ophthalmologist diagnosed me with excessive secretion from the sebaceous glands.
Now, I am experiencing more frequent yellow, thick discharge, and I have developed a white dot on my lower right eyelid.
My eye also feels unusually painful.
Is this also a manifestation of excessive secretion?

Dr. Wang Ziyang reply Ophthalmology

The issue of eye irritation may be due to seborrheic conjunctivitis, while the white spots on the eyelids could be cysts of the sebaceous glands or meibomian glands.
It is also possible that these white spots are the so-called "whiteheads" associated with acne, which may not necessarily be related to the eye irritation.

More Info

When it comes to the appearance of white bumps on the lower eyelid, there are several potential causes that can be considered. Based on your description, it sounds like you may be experiencing a condition related to the oil glands in your eyelids, commonly known as meibomian glands. These glands are responsible for producing the oily layer of your tears, which helps to keep your eyes lubricated.
The white bump you are noticing could be a chalazion or a stye (hordeolum). A chalazion is a painless lump that can develop when a meibomian gland becomes blocked. It often appears as a firm, round bump on the eyelid and can sometimes cause discomfort or irritation. On the other hand, a stye is usually caused by a bacterial infection of the oil glands, leading to a red, swollen, and painful bump at the edge of the eyelid.
The yellow, thick discharge you mentioned could indicate an underlying infection or inflammation, particularly if it is accompanied by pain and irritation. This could be related to blepharitis, which is an inflammation of the eyelid margins often caused by seborrheic dermatitis or bacterial overgrowth. In cases of blepharitis, the eyelids can become crusty, and there may be an increase in discharge due to the inflammation of the glands.

Given your history of excessive secretion from the sebaceous glands, it is possible that your eyelid condition is related to this issue. The increased production of oil can lead to blockages in the glands, resulting in the formation of bumps. Additionally, if there is a bacterial component to the discharge, it may exacerbate the condition, leading to further irritation and pain.

It is important to address these symptoms with a healthcare professional, preferably an ophthalmologist, who can perform a thorough examination of your eyes and eyelids. They may recommend warm compresses to help alleviate the blockage, eyelid hygiene practices to reduce inflammation, or even prescribe antibiotic ointments if an infection is suspected.
In the meantime, you can try to keep the area clean by gently washing your eyelids with a mild soap or eyelid scrub. Avoid touching or rubbing your eyes, as this can introduce more bacteria and worsen the condition. If the bump does not improve or if you experience worsening symptoms, such as increased pain, swelling, or vision changes, seek medical attention promptly.

In summary, the white bump on your lower eyelid could be a chalazion or stye, potentially related to your history of excessive oil secretion. The yellow discharge and pain suggest that there may be an underlying infection or inflammation that needs to be addressed. Consulting with an eye care professional will provide you with the best course of action for diagnosis and treatment.

There are some small white spots on the lower eyelid that are not painful or itchy, but they are somewhat unsightly. Recently, a pimple developed under the eyelid, but it later turned into a white bump. What could this be, what does it mean, and how can it be removed?

Dr. Huang Bozhen reply Ophthalmology
Since there is no pain or discomfort, it should not cause any issues. Daily warm compresses may be helpful. Poor secretion from the sebaceous glands can lead to the appearance of small white spots, and in severe cases, it may result in symptoms of dry eye syndrome.
