Foreskin issues
Hello, Dr.
Hsu.
My family asked me to get a circumcision today! Indeed! When the penis is erect, it feels tight around the shaft! When not erect, the foreskin extends about two centimeters! Moreover, from the outside, it only vaguely resembles the shape of the glans! Doctor, may I ask you, after the circumcision, will the penis be completely exposed? Will it appear larger? I am quite overweight.
Also, how much does the circumcision procedure cost? Will there be a nurse present? I'm afraid I might feel embarrassed! Can I just tell the urologist that I want a circumcision? How long does the surgery take, and how long is the recovery period? Is there a laser option available?

Hello: After a circumcision, the penis will be exposed.
The cost of the surgery varies by hospital, typically ranging from NT$5,000 to NT$8,000, and it is usually out-of-pocket.
If the patient is overweight, it may be necessary to remove some fat from the base of the penis for better results.
The surgery takes about 15 to 30 minutes, but if liposuction is included, it may take up to 90 minutes, and anesthesia and hospitalization (preferably) are required.
It is generally recommended to avoid sexual activity for two months post-surgery.
Some hospitals offer laser surgery, which is more expensive but results in a neater wound.

More Info
Circumcision is a surgical procedure that involves the removal of the foreskin, which is the fold of skin covering the glans (the head) of the penis. If you are considering this procedure, it’s important to understand what to expect before and after surgery, as well as the potential benefits and risks involved.
Before the Surgery
1. Consultation: You will first have a consultation with a urologist or surgeon. During this visit, you can discuss your concerns, the reasons for the procedure, and what to expect. It’s perfectly acceptable to express any fears or questions you may have.
2. Physical Examination: The doctor will perform a physical examination to assess your condition. This may include checking the tightness of your foreskin and any other relevant factors.
3. Preoperative Instructions: You will receive instructions on how to prepare for the surgery. This may include fasting for a certain period before the procedure and avoiding certain medications that can increase bleeding.
4. Cost and Insurance: The cost of circumcision can vary widely depending on your location, the surgeon’s fees, and whether you have insurance coverage. It’s advisable to check with your insurance provider to see if the procedure is covered.
5. Anesthesia: Circumcision is typically performed under local anesthesia, which numbs the area, or general anesthesia, where you will be asleep during the procedure. Discuss with your doctor which option is best for you.
The Surgery
1. Duration: The procedure usually takes about 30 minutes to an hour.
2. Setting: Circumcision is often performed in an outpatient setting, meaning you can go home the same day. There will be medical staff present, including nurses, to assist during the procedure and ensure your comfort.
3. Laser Circumcision: Some facilities may offer laser circumcision, which can reduce bleeding and promote faster healing. However, traditional surgical methods are also very effective.
After the Surgery
1. Immediate Recovery: After the procedure, you will be monitored for a short time to ensure there are no immediate complications. You may experience some swelling, discomfort, or bleeding, which is normal.
2. Postoperative Care: You will receive instructions on how to care for the surgical site. This typically includes keeping the area clean, applying ointment, and avoiding strenuous activities for a few weeks.
3. Healing Time: Full recovery can take about 4 to 6 weeks, but many patients feel comfortable returning to normal activities within a week or two.
4. Appearance: After circumcision, the glans will be fully exposed, and you may notice some changes in sensitivity. The penis may appear larger due to the removal of the foreskin, but this is often subjective and can vary from person to person.
5. Follow-Up: A follow-up appointment may be scheduled to check on the healing process and address any concerns you might have.
Emotional Considerations
It’s completely normal to feel anxious or shy about the procedure. Remember that medical professionals are trained to handle such situations with sensitivity and confidentiality. If you have concerns about modesty, you can discuss this with your healthcare provider beforehand.
In conclusion, circumcision can provide benefits such as improved hygiene and reduced risk of certain infections. However, it’s essential to weigh these benefits against any potential risks and to have a thorough discussion with your healthcare provider to make an informed decision.
