Chronic nasal congestion
Hello, Doctor: I underwent surgery for inferior turbinate hypertrophy and a deviated septum about two and a half years ago.
Initially, my nasal congestion improved, but after about six months, it started to return, and now I experience nasal congestion almost all day.
Although the congestion occurs on both sides, it is primarily concentrated on the right side, with the left side only occasionally affected.
Should this be considered unilateral nasal congestion? Is it possible that the turbinates have thickened again, or could there be other issues? Do I need to seek medical evaluation? Thank you! ^___________^

Hello: It is possible that the nasal turbinates have hypertrophied again or there may be other issues causing nasal congestion.
I recommend that you seek medical evaluation.
Sincerely, Hook Sin.

More Info
Chronic nasal congestion can be a frustrating and debilitating condition, especially when it interferes with daily activities and sleep. Based on your description, it seems that you have undergone surgery for inferior turbinate hypertrophy and a deviated septum about two and a half years ago, which initially provided some relief. However, the recurrence of nasal congestion, particularly concentrated on one side, raises several important considerations.
Understanding Chronic Nasal Congestion
Chronic nasal congestion can arise from various causes, including:
1. Anatomical Issues: After surgery, it is possible for the nasal structures to change over time. The inferior turbinates may become hypertrophied again, leading to obstruction. Additionally, if the septum has shifted or if there are new anatomical changes, this could contribute to your symptoms.
2. Allergic Rhinitis: Allergies are a common cause of nasal congestion. If you are exposed to allergens such as pollen, dust mites, or pet dander, your nasal mucosa may become inflamed, leading to congestion. This can occur even if you have had surgery.
3. Chronic Sinusitis: Inflammation of the sinuses can lead to nasal congestion, especially if there is a history of recurrent sinus infections. Symptoms may include facial pain, pressure, and thick nasal discharge.
4. Nasal Polyps: These are non-cancerous growths that can develop in the nasal passages or sinuses, often associated with chronic inflammation. They can obstruct airflow and lead to persistent congestion.
5. Vasomotor Rhinitis: This condition is characterized by nasal congestion that is not due to allergies or infections. It can be triggered by environmental factors such as changes in temperature, humidity, or exposure to irritants like smoke or strong odors.
Solutions and Recommendations
Given your history and current symptoms, here are some steps you can take:
1. Consult an Ear, Nose, and Throat (ENT) Specialist: Since you are experiencing persistent nasal congestion, it is advisable to see an ENT specialist for a thorough evaluation. They may perform a nasal endoscopy to visualize the nasal passages and identify any anatomical issues, polyps, or other abnormalities.
2. Allergy Testing: If allergies are suspected, consider undergoing allergy testing. Identifying specific allergens can help in managing symptoms through avoidance strategies or medications.
3. Medical Management: Depending on the findings, your doctor may recommend various treatments, including:
- Intranasal Corticosteroids: These can help reduce inflammation in the nasal passages and alleviate congestion.
- Antihistamines: If allergies are a contributing factor, antihistamines can help relieve symptoms.
- Saline Nasal Irrigation: Regularly using saline sprays or performing nasal irrigation can help keep the nasal passages moist and clear out mucus.
4. Surgical Options: If conservative measures fail and anatomical issues are identified, surgical options may be considered. This could involve revision surgery to address any structural problems that have developed since your initial procedure.
5. Lifestyle Modifications: Avoiding known irritants, using a humidifier, and maintaining good hydration can also help manage symptoms.
Conclusion
In summary, your experience of chronic nasal congestion, particularly with a focus on one side, warrants further investigation. While it is possible that the inferior turbinate has become hypertrophied again or that other factors are at play, a comprehensive evaluation by an ENT specialist will provide clarity. They can guide you through the appropriate diagnostic tests and treatment options tailored to your specific situation. Remember, addressing chronic nasal congestion not only improves your quality of life but also enhances overall health and well-being.
