Post-abortion bleeding

On January 1st, I had an abortion, and since then, I have been experiencing bleeding, varying in amount.
On February 5th, I went to a large hospital for an examination and blood tests.
The doctor said there were no significant issues and prescribed medication to stop the bleeding.
However, last night (February 13th), I started bleeding again.
What could be the problem? (I have also had sexual intercourse during this time!)

Xiao to, 10~19 year old female. Ask Date: 2007/02/14

Under normal circumstances, after a 1/1 abortion, the next menstruation is expected around 2/5.
However, if there is intermittent bleeding in between, the menstruation may be delayed, but it is possible that menstruation could start on 2/13.

Post-abortion bleeding can be a concerning issue for many women, and understanding the causes and when to seek medical help is crucial for ensuring your health and well-being. After an abortion, it is common to experience some bleeding as the body begins to heal. However, the nature and duration of this bleeding can vary significantly from person to person.

In the first few days following an abortion, light bleeding or spotting is expected. This is due to the shedding of the uterine lining and the body’s adjustment to the hormonal changes that occur after the procedure. However, if the bleeding persists for an extended period, becomes heavier, or is accompanied by severe pain, fever, or foul-smelling discharge, it may indicate a complication that requires medical attention.

There are several potential causes of prolonged bleeding after an abortion:
1. Incomplete Abortion: Sometimes, not all of the pregnancy tissue is expelled from the uterus, which can lead to ongoing bleeding. This may require a follow-up procedure, such as a dilation and curettage (D&C), to remove any remaining tissue.

2. Infection: An infection in the uterus can cause prolonged bleeding and may be accompanied by other symptoms such as fever, chills, and abdominal pain. If you suspect an infection, it is essential to seek medical care promptly.

3. Hormonal Changes: After an abortion, your body undergoes significant hormonal shifts, which can affect your menstrual cycle and lead to irregular bleeding. This bleeding may be lighter or heavier than your usual menstrual flow.

4. Uterine Atony: This condition occurs when the uterus does not contract effectively after the abortion, which can lead to excessive bleeding. Medications may be prescribed to help the uterus contract and reduce bleeding.

5. Retained Products of Conception: If any tissue remains in the uterus, it can cause continued bleeding and may require further medical intervention.

In your case, since you experienced bleeding after your abortion on January 1st and were reassured by your doctor on February 5th, it is important to monitor the situation closely. The fact that you have started bleeding again on February 13th, especially after engaging in sexual activity, warrants further evaluation. It is advisable to contact your healthcare provider to discuss your symptoms, as they may want to perform an ultrasound or other diagnostic tests to rule out any complications.

When to Seek Help:
- If you experience heavy bleeding (soaking through one or more pads in an hour for two consecutive hours).

- If you have severe abdominal pain that is not relieved by over-the-counter pain medications.

- If you develop a fever (temperature over 100.4°F or 38°C).

- If you notice a foul-smelling discharge.

- If you feel lightheaded, dizzy, or faint.

In conclusion, while some bleeding after an abortion is normal, persistent or heavy bleeding can indicate a problem that needs medical attention. It is always better to err on the side of caution and consult your healthcare provider if you have concerns about your symptoms. They can provide you with the appropriate care and guidance to ensure your recovery is as smooth as possible.
