Please help me!
Hello Doctor,
1.
I have been experiencing chest pain and tightness, sometimes accompanied by tinnitus and dizziness.
The chest pain and tightness persist throughout the day, and I have noticed a decline in my physical strength and increased fatigue.
A few years ago, I underwent surgery for hyperhidrosis, and after suffering from acute gastroenteritis in early February this year, my appetite has decreased.
After eating, I often experience excessive burping, which is uncomfortable, and sometimes I wake up at night to burp, feeling like I can't breathe.
2.
I have been taking gastrointestinal medication, but after taking it, I experience scalp numbness.
Even when I don't take the medication, I still feel numbness.
Could this side effect affect my brain? The medications prescribed by my doctor include loratadine 10 mg, D.M.P 10 mg, metoclopramide (Primperan) 4.5 mg, Gascon 40 mg, pseudoephedrine 60 mg, and codeine syrup (4% concentration) 120 ml.
3.
For the past few months, I have been consulting various doctors to find the cause of my symptoms.
I have seen a cardiologist and a gastroenterologist, who performed an ultrasound that showed normal gastric wall condition, and a pulmonologist who conducted an X-ray that appeared clean and normal.
Finally, I even consulted a psychiatrist, but I am certain that my issues are not related to mental health.
Out of desperation, I started taking psychiatric medication, but my mental state has continued to deteriorate, and the aforementioned symptoms have not improved.
Doctor, please help me!

It is likely a problem with the autonomic nervous system, and consulting a neurologist would also be appropriate.

It sounds like you are experiencing a complex set of symptoms that can be quite distressing. Your symptoms of chest pain, chest tightness, dizziness, and fatigue, along with gastrointestinal issues such as bloating and excessive belching, warrant a thorough evaluation. Given your history of acute gastroenteritis and the various medications you have been prescribed, it is essential to consider both gastrointestinal and potential non-gastrointestinal causes for your symptoms.
Firstly, the chest pain and tightness you are experiencing could be related to several factors. While it is good that you have seen specialists in cardiology and pulmonology, it is important to rule out any cardiac issues, especially since chest pain can sometimes be a sign of heart problems. However, since your tests have come back normal, we can consider other possibilities. Gastroesophageal reflux disease (GERD) is a common condition that can cause chest discomfort, especially after eating, and can lead to symptoms such as belching and a sensation of not being able to breathe. The fact that you experience increased belching after meals and have a reduced appetite could suggest that your gastrointestinal tract is not functioning optimally.
The medications you mentioned, including metoclopramide, are often used to treat nausea and promote gastric emptying. However, they can also have side effects, including dizziness and a sensation of tingling or numbness, which you described as a "numbness of the scalp." This side effect is not common but can occur in some individuals. It is crucial to communicate these side effects to your doctor, as they may need to adjust your medication regimen.
Your symptoms of dizziness and fatigue could also be linked to anxiety or stress, especially if you have been feeling overwhelmed by your health issues. Anxiety can manifest physically, leading to symptoms such as chest tightness, dizziness, and fatigue. It is essential to address both the physical and psychological aspects of your health. If you feel that the medications prescribed by the psychiatrist are not helping, it may be worth discussing alternative treatments or therapies, such as cognitive-behavioral therapy (CBT), which can be effective for managing anxiety and stress-related symptoms.
In terms of your gastrointestinal symptoms, it may be beneficial to keep a food diary to identify any potential triggers for your bloating and belching. Certain foods, especially those high in fat or sugar, can exacerbate these symptoms. Additionally, consider incorporating smaller, more frequent meals into your diet, as large meals can put additional pressure on your digestive system.
Given the complexity of your symptoms, it may be worthwhile to seek a multidisciplinary approach to your care. This could involve working with a gastroenterologist, a cardiologist, and a mental health professional to ensure that all aspects of your health are being addressed. If your symptoms persist despite treatment, further diagnostic testing, such as an upper endoscopy or pH monitoring, may be warranted to evaluate for conditions like GERD or functional dyspepsia.
In summary, your symptoms are multifaceted and may require a comprehensive approach to diagnosis and treatment. Open communication with your healthcare providers about your symptoms, medication side effects, and any concerns you have is vital for finding an effective treatment plan. Remember, you are not alone in this journey, and there are resources and support available to help you manage your symptoms and improve your quality of life.
