Two questions?
1.
Hello doctor, I used to masturbate with my foreskin intact.
I was considering getting a circumcision, but in the past few months, I have tried retracting the foreskin, and now it tends to retract on its own while I sleep.
Do I still need to get circumcised?
2.
Additionally, ever since my foreskin became looser, I’ve noticed that when I masturbate, the area just below the glans, where it connects to the foreskin, feels more sensitive and makes me want to ejaculate more than the glans itself.
Isn’t the glans usually more pleasurable? Is this normal? Could it be a sign of premature ejaculation?

Dr. Xu Weikai reply Urology
Hello,
1.
You can retract the foreskin; surgery for phimosis is generally not necessary.
2.
For most patients with phimosis, sensitivity typically increases after retracting the foreskin.
After some time, you will get used to it.
Additionally, the sensitive areas are not limited to the glans; they can vary from person to person.
Sincerely,
Dr.

When it comes to understanding foreskin sensitivity and the decision regarding circumcision, it's important to consider both anatomical and physiological aspects. Let's address your questions one by one.
1. Do you still need to get circumcised if the foreskin can retract?
If you have successfully retracted your foreskin and it can remain behind the glans (the head of the penis) comfortably, circumcision may not be necessary. The primary reasons for circumcision often include recurrent infections, phimosis (the inability to retract the foreskin), or personal preference. If you are not experiencing any pain, discomfort, or recurrent infections, and you can maintain good hygiene, then circumcision might not be required. However, it is always advisable to consult with a urologist or healthcare provider to discuss your specific situation and any potential risks or benefits associated with circumcision.
2. Is it normal to feel more sensitivity in the area where the foreskin connects to the glans?
Yes, it is quite normal for some individuals to experience heightened sensitivity in the area where the foreskin connects to the glans, known as the frenulum. This area can be particularly sensitive due to a higher concentration of nerve endings. Many men find that the frenulum can elicit a strong sexual response, sometimes even more so than the glans itself. This does not necessarily indicate a problem or early signs of premature ejaculation. Sensitivity varies greatly among individuals, and what you are experiencing is within the range of normal sexual response.
Understanding Sensitivity and Sexual Health:
Sensitivity in the genital area can be influenced by various factors, including hormonal levels, psychological state, and individual anatomy. If you find that your sensitivity is affecting your sexual performance or leading to concerns about premature ejaculation, it may be beneficial to explore techniques for managing arousal and ejaculation. These can include behavioral techniques, such as the start-stop method or the squeeze technique, which can help you gain better control over your ejaculation.
When to Seek Further Evaluation:
If you notice any significant changes in your sexual function, such as persistent pain, discomfort, or changes in sensitivity that concern you, it is important to seek medical advice. A healthcare provider can perform a thorough evaluation and provide guidance tailored to your specific needs. Additionally, if you have any concerns about sexually transmitted infections (STIs) or other urological issues, it is wise to get tested and discuss these with your doctor.
In conclusion, your current situation regarding foreskin retraction and sensitivity appears to be normal. However, maintaining open communication with your healthcare provider is crucial for addressing any ongoing concerns and ensuring your sexual health remains optimal.
