Postoperative
Hello doctor, I had surgery for penile curvature four months ago.
The incision was made in a circular pattern beneath the glans, and I did not undergo a circumcision.
The wound has healed, but the area of the foreskin (which is in front of the incision) has been swollen since the surgery.
Although the swelling has decreased compared to before, it is still thicker than normal, and the prominent ring of foreskin is quite noticeable.
I have been returning for follow-up appointments every month after the surgery, and the doctors have told me that the swelling will gradually subside.
They mentioned that my lymphatic drainage might be slower and that some blood vessels need time to heal.
My last check-up was at the end of last month, and the doctor said I am no longer swollen and do not need to return for further visits.
However, I feel that my condition is still not normal compared to others, and I am feeling very discouraged.
I wish I had not undergone the surgery in the first place.
What should I do? Should I consider having a circumcision? I am worried that it might worsen the situation.
Additionally, I still experience some pain when pressing on the scar.
Is this due to hypertrophic scarring or unresolved edema? I am very troubled.
Which specialist should I consult to address these issues?

Dr. Huang Zhihui reply Surgery
Post-surgical wounds may experience some swelling and pain due to the scar tissue repair response, which is a common phenomenon.
It often takes more than a year for the wound to fully flatten and resemble normal tissue.
It is advisable to consult a urologist for an evaluation; they will quickly be able to determine the cause.

More Info
Post-surgery concerns, particularly after penile surgery, can be quite distressing, especially when it comes to swelling and scar tissue management. It’s understandable to feel anxious about the changes in your body following a surgical procedure, particularly one as sensitive as penile surgery.
From your description, it seems that you underwent a surgery to correct penile curvature, which involved making an incision around the glans (the head of the penis). Post-operative swelling is a common occurrence after such surgeries, and it can take time for the body to heal completely. The swelling you are experiencing in the area of the foreskin (even though you did not have a circumcision) may be due to a combination of factors, including fluid retention, inflammation, and the healing process itself.
Understanding Swelling and Scar Tissue
1. Swelling: After surgery, it is normal for the body to react with inflammation, which can lead to swelling. This is part of the healing process as the body sends blood and nutrients to the area to promote recovery. The lymphatic system also plays a role in draining excess fluid, and sometimes this process can be slower in certain individuals, leading to prolonged swelling.
2. Scar Tissue: Scar formation is a natural part of the healing process. However, some individuals may develop hypertrophic scars or keloids, which are raised and can be more prominent than the surrounding skin. The pain you are experiencing when pressing on the scar could indicate that the scar tissue is still maturing or that there is some residual inflammation.
Recommendations for Management
1. Follow-Up with Your Surgeon: It’s crucial to maintain open communication with your surgeon. If you feel that your concerns are not being adequately addressed, consider seeking a second opinion from another qualified urologist or surgeon who specializes in penile surgeries. They can provide a fresh perspective on your situation.
2. Physical Therapy: Some patients benefit from physical therapy focused on scar management. Techniques such as massage therapy can help break down scar tissue and improve circulation, which may aid in reducing swelling and discomfort.
3. Topical Treatments: There are various topical treatments available that can help with scar management. Silicone gel sheets or silicone ointments are often recommended to help flatten and soften scars. Additionally, corticosteroid creams may be prescribed to reduce inflammation and itching.
4. Avoiding Further Surgery: While it might be tempting to consider additional surgery, such as a circumcision, it’s essential to weigh the risks and benefits. Additional surgery could potentially lead to more complications or dissatisfaction. Discuss this thoroughly with your healthcare provider.
5. Monitoring Changes: Keep a close eye on the swelling and any changes in the scar. If you notice increased pain, redness, or any signs of infection, it’s important to seek medical attention promptly.
6. Psychological Support: It’s not uncommon to feel a sense of distress or disappointment after surgery, especially if the results are not what you expected. Speaking with a mental health professional or a support group can provide emotional support and coping strategies.
Conclusion
In summary, while it can be frustrating to deal with prolonged swelling and scar tissue after penile surgery, it’s important to remember that healing takes time. Continue to follow up with your healthcare provider, consider supportive therapies, and be patient with your body as it recovers. If you feel that your concerns are not being addressed, do not hesitate to seek a second opinion. Your comfort and well-being are paramount, and there are options available to help you manage your post-surgical experience.
