Upper abdominal pain after sterilization?

Hello, doctor...
I underwent a tubal ligation surgery in the obstetrics and gynecology department two days ago.
The incision site has quickly stopped hurting, but I have been experiencing persistent pain in my upper abdomen, even when I burp (in the area just below the rib cage).
My abdomen feels very bloated, and despite eating very lightly, I have been having diarrhea for two days post-surgery.
Could you please explain why this is happening?

Helen, 30~39 year old female. Ask Date: 2007/07/29

Dr. Chen Fuhao reply Obstetrics and Gynecology

Dear Helen,
The incision from the sterilization procedure has quickly stopped hurting, but the upper abdomen continues to be very painful.
I have persistent bloating and diarrhea, and I am unsure if this is due to a medication allergy (such as antibiotics or pain relievers).
I recommend that you consult your original obstetrician for further evaluation.
Wishing you a speedy recovery.

Post-sterilization upper abdominal pain can be concerning, especially when accompanied by symptoms like bloating and diarrhea. After a tubal ligation, it's not uncommon to experience some discomfort, but persistent or severe pain warrants further investigation.

There are several potential causes for your upper abdominal pain following the procedure. First, it's important to consider that during a tubal ligation, the surgeon manipulates the reproductive organs, which can lead to temporary inflammation or irritation in the surrounding tissues. This can manifest as pain in the upper abdomen, particularly if there was any manipulation of the uterus or nearby structures.

Another possibility is that the pain could be related to gas or bloating. After surgery, especially if you have been under anesthesia, your gastrointestinal tract may take some time to return to normal function. This can lead to gas buildup, which can cause discomfort and pain, particularly in the upper abdomen. Additionally, if you are experiencing diarrhea, this could further contribute to abdominal discomfort as your body processes food differently post-surgery.

It's also essential to consider the possibility of complications, such as infection or internal bleeding. While these are less common, they can occur after any surgical procedure. Signs of infection may include fever, increased pain, or discharge from the surgical site. If you notice any of these symptoms, it is crucial to seek medical attention promptly.

Furthermore, the stress and anxiety surrounding surgery can also exacerbate physical symptoms. Anxiety can lead to increased muscle tension and gastrointestinal disturbances, which may contribute to your abdominal pain and bloating. If you have a history of anxiety, it might be beneficial to address this with your healthcare provider, as they may recommend strategies to help manage your symptoms.

Given that you are experiencing persistent pain and gastrointestinal symptoms two days post-surgery, it is advisable to follow up with your healthcare provider. They may want to perform a physical examination, possibly including imaging studies, to rule out any complications. It’s essential to communicate all your symptoms clearly, including the location and nature of the pain, any changes in bowel habits, and any other associated symptoms.

In summary, while some discomfort after a tubal ligation is expected, persistent upper abdominal pain, especially when accompanied by bloating and diarrhea, should be evaluated by a healthcare professional. They can help determine the underlying cause and ensure that you receive appropriate care and management. Remember, your health and well-being are paramount, and seeking medical advice is always a prudent step when you have concerns following a surgical procedure.
