Suddenly developing a large number of acne lesions..?

Hello Dr.
Hong: I am someone who has not had much acne since puberty, but recently I have been experiencing many small pimples on my back, buttocks, face, and under my ears.
They feel like an allergic reaction and are also itchy.
I visited a dermatologist who diagnosed me with acne folliculitis and prescribed three topical medications: 1) Clindamycin wash, 2) Clindamycin gel, and 3) Erythromycin gel 0.1%.
I was also given oral medications: 1) Doxycycline 100 mg, 2) Trand 250 mg, and 3) Frotin s.c.
250 mg.
However, I feel like none of these treatments are effective, and the pimples keep multiplying.
I have never experienced anything like this before.
Could it be that this is not acne but another condition that presents similar symptoms? Should I take something else to stop this? Could it be due to contaminated water? Since I have never had this issue before, I wonder if it is related to my diet.
Please advise me on what to do...
I am really scared that it will keep getting worse and won't get better.
Am I seeing the wrong specialist?

Dr. Hong Zhengsheng reply Dermatology

Hello Momo: The best advice for you is to find a dermatologist you trust and consult with them.
I apologize, but based on your description, I cannot determine your specific condition.
Answers found online can only speculate based on the information provided and personal experience; they cannot provide the precise diagnosis and treatment that an in-person consultation can offer.
Therefore, I recommend that when you notice the symptoms described, you should promptly visit a public hospital in your city or county and consult with a dermatology specialist.
All the physicians here are very willing to assist you.

Hello! It sounds like you're experiencing a frustrating and distressing situation with your sudden acne breakouts. Let's break down the potential causes, treatments, and when you should seek further help.

Causes of Sudden Acne Breakouts
1. Hormonal Changes: Fluctuations in hormones, particularly androgens, can lead to increased oil production in the skin, which can clog pores and result in acne. This is common during puberty, menstrual cycles, pregnancy, or even stress.

2. Dietary Factors: Certain foods, especially those high in sugar and dairy, can exacerbate acne in some individuals. If you've recently changed your diet, this could be a contributing factor.

3. Skin Care Products: Sometimes, the products you use on your skin can lead to breakouts, especially if they are comedogenic (pore-clogging). It's important to use non-comedogenic products if you're prone to acne.

4. Environmental Factors: Exposure to pollutants, humidity, or even changes in water quality can affect your skin. If you've recently moved or changed your environment, this could be a factor.

5. Underlying Health Issues: Conditions such as polycystic ovary syndrome (PCOS) or other hormonal imbalances can lead to acne. If your acne is persistent and severe, it might be worth investigating these possibilities.

Treatments
You mentioned that you've been prescribed several topical treatments and oral medications. Here’s a brief overview of what they do:
- Cleansers (如可麗敏洗劑): These help to remove excess oil and bacteria from the skin's surface.

- Topical Gels (如卡達凝膠和愛芙琳凝膠): These typically contain ingredients like benzoyl peroxide or retinoids that help to reduce inflammation and prevent clogged pores.

- Oral Antibiotics (如doxymycin): These are used to reduce bacteria and inflammation associated with acne.

- Hormonal Treatments (如trand): These can help regulate hormonal fluctuations that may be contributing to your acne.

If these treatments haven't been effective, it may be time to consider other options:
1. Consult a Dermatologist Again: If your acne persists despite treatment, it may be beneficial to revisit your dermatologist. They can reassess your condition and possibly recommend alternative treatments, such as hormonal therapy or isotretinoin for severe cases.

2. Consider Lifestyle Changes: Evaluate your diet, skincare routine, and stress levels. Keeping a food diary may help identify any dietary triggers.

3. Explore Other Conditions: If your symptoms resemble allergies or other skin conditions (like folliculitis or contact dermatitis), a dermatologist may perform tests to rule these out.

When to Seek Help
You should seek further medical advice if:
- Your acne worsens or spreads despite treatment.

- You experience significant pain, swelling, or signs of infection (like pus or fever).

- You have concerns about scarring or emotional distress related to your acne.

Conclusion
Acne can be a complex condition influenced by various factors, and it’s understandable to feel overwhelmed. It’s important to communicate openly with your healthcare provider about your concerns and treatment efficacy. They can help tailor a treatment plan that works for you, and if necessary, refer you to a specialist for further evaluation. Remember, managing acne often requires patience and persistence, but with the right approach, improvement is possible.
