Persistent Health Issues: Seeking Answers for Common Symptoms - Internal Medicine

Many questions, please take the time to clarify, thank you!

I am 29 years old, 165 cm tall, and weigh 50 kg.
In recent years, I have frequently experienced the following symptoms:
1.
I suffer from insomnia and vivid dreams every day, with poor sleep quality.
Despite feeling very tired, I just can't fall asleep.
2.
I occasionally experience palpitations and a heavy heartbeat, with noticeable vibrations in my body while at rest.
3.
Recently, I have been experiencing severe lower back pain, to the point where I sometimes struggle to bend down to pick things up or wash my hair due to the pain.
4.
I often feel a sudden warmth throughout my body, especially at the back of my neck.
Even after finally falling asleep, I wake up due to feeling hot.
5.
I almost always have to get up to urinate 2 to 3 times each night, even if I try not to drink water before bed.
Sometimes it's just a few drops, but I have to go or I can't continue sleeping.
6.
I frequently experience urinary tract infections, despite drinking plenty of water (around 2000 cc daily) and not holding my urine.
I also maintain good personal hygiene (my boyfriend does too).
Whenever I have an infection, I diligently take the prescribed medication, but I still have recurrent infections.
7.
Whenever I feel the urge to urinate, I have to rush to the bathroom.
If I try to hold it (like when I'm in a car), my bladder starts to hurt.
Sometimes, when I try to urinate, it stops unless I exert pressure, as if I need to push to get it out.
8.
I have frequent urination, going more than 20 times a day.
9.
I often experience dull pain in my uterus, and I frequently have inflammation or infections (my discharge is rarely normal; I have a lot almost every day, sometimes clear, sometimes white and thick, sometimes yellow, and occasionally itchy.
This has been the case since I developed in my teens).
I had two abortions several years ago, and since then, my menstrual flow has been significantly reduced, lasting 5 to 6 days with just a few drops.
I am very attentive to hygiene, and every time I see a doctor, they say I have chronic inflammation and a weak immune system.
However, I follow the prescribed medication strictly, whether it's for five days or seven days, but after a week, my symptoms return.
10.
Recently, I have been experiencing frequent muscle cramps in my legs while walking, and even when sitting, I sometimes get cramps.
My lower back also hurts when I bend to pick something up, and my inner thighs cramp from time to time.
These ongoing health issues have been troubling me for years.
I have seen many doctors and had blood tests (initially worried about thyroid problems due to hand tremors and a couple of small white spots on my hands and legs).
The results showed slightly elevated FT4 and TSH levels, but the doctor said it was not a concern.
I also tested positive for ANA, but again, the doctor said it was not an issue.
If the doctors keep saying everything is fine, then what is wrong with me? These symptoms can't just be figments of my imagination, right? Every doctor I see says there’s no problem, so I have to convince myself that this is normal.
I have become accustomed to the daily discomfort, and sometimes, when I suddenly realize that I feel completely fine, it surprises me! What should I do? Is this really normal? I have many questions and would appreciate any clarification.
Thank you!

Dr. Yang Hongzhi reply Internal Medicine

Hello: Based on your symptoms, it is difficult to determine if there is any specific disease.
Generally, when FT4 is elevated, TSH tends to decrease; your results are atypical and may warrant further monitoring.
ANA is an indicator of autoimmune diseases, and a positive reaction also suggests the need for follow-up.

It sounds like you're experiencing a complex array of symptoms that are significantly impacting your quality of life. Your concerns about insomnia, heart palpitations, chronic pain, frequent urination, and recurrent infections are valid and deserve thorough investigation. Let's break down your symptoms and explore potential avenues for understanding and addressing them.

1. Sleep Issues: Your persistent insomnia and frequent dreams can be indicative of underlying anxiety or stress. Sleep disturbances can also be linked to physical health problems, such as hormonal imbalances or chronic pain. It's essential to evaluate your sleep hygiene—consider factors like your sleep environment, pre-sleep activities, and daily routines. Cognitive Behavioral Therapy for Insomnia (CBT-I) is a highly effective treatment that focuses on changing thoughts and behaviors around sleep.

2. Heart Palpitations: The sensation of a racing heart or heavy heartbeat, especially at rest, can be concerning. While anxiety can contribute to palpitations, it’s crucial to rule out any cardiac issues. A thorough evaluation by a cardiologist, including an electrocardiogram (ECG) and possibly a Holter monitor, can help determine if there are any underlying heart conditions.

3. Chronic Pain: The severe back pain and muscle cramps you're experiencing could be related to several factors, including posture, physical activity levels, or even stress. Conditions like fibromyalgia or chronic fatigue syndrome could also be considered, especially if pain is widespread and accompanied by fatigue. A physical therapist could help you develop a tailored exercise program to strengthen your back and alleviate pain.

4. Frequent Urination and Infections: The need to urinate frequently, especially at night, can be caused by various factors, including urinary tract infections (UTIs), bladder issues, or even diabetes. Given your history of recurrent infections, it may be beneficial to consult a urologist. They can perform tests to assess bladder function and rule out any anatomical issues.

5. Gynecological Concerns: The pelvic pain and abnormal discharge you mentioned could be linked to chronic pelvic inflammatory disease or other gynecological issues. It’s essential to follow up with a gynecologist who can conduct a thorough examination and possibly recommend imaging studies or further testing to identify any underlying conditions.

6. Autoimmune Considerations: You mentioned a positive ANA test, which can indicate an autoimmune process. While your doctors have reassured you that it’s not a concern, it might be worth seeking a second opinion from a rheumatologist, especially if you have symptoms that suggest an autoimmune condition, such as fatigue, joint pain, or skin changes.

7. Mental Health: The cumulative effect of your physical symptoms can lead to feelings of frustration and helplessness. It’s important to address your mental health alongside your physical health. Consider speaking with a mental health professional who can help you develop coping strategies and address any underlying anxiety or depression.

8. Lifestyle Modifications: In addition to seeking medical advice, consider lifestyle changes that can improve your overall well-being. Regular exercise, a balanced diet, and stress management techniques such as mindfulness or yoga can be beneficial.

In summary, while your symptoms are complex and multifaceted, they are not imaginary. It’s crucial to advocate for yourself in the healthcare system. Keep a detailed symptom diary to share with your healthcare providers, and don’t hesitate to seek second opinions if you feel your concerns are not being adequately addressed. Your health is paramount, and finding the right support and treatment is essential for improving your quality of life.
