Premature ejaculation?
It's been over a year since I've had sex.
I recently started dating a girl, but now my glans has become very sensitive.
Every time I try to penetrate, I feel like I want to ejaculate right away.
Also, it seems like I can't maintain an erection for very long.
What could be the reason for this? Can I use self-training to reduce sensitivity?
TATA, 20~29 year old female. Ask Date: 2007/08/26
Dr. Xu Weikai reply Urology
Tata: Hello, this is how it is; if there is not frequent stimulation, sensitivity will increase, leading to phenomena such as premature ejaculation.
Self-training can indeed reduce sensitivity and prolong erection time.
This is a response from Dr.

More Info
Premature ejaculation (PE) is a common sexual dysfunction that affects many men at some point in their lives. It is characterized by the inability to control ejaculation during sexual intercourse, leading to ejaculation that occurs sooner than desired. This can result in distress for both partners and can affect the overall sexual experience.
Causes of Premature Ejaculation
1. Psychological Factors: Anxiety, stress, and relationship issues can significantly contribute to PE. Performance anxiety, in particular, can create a cycle where the fear of premature ejaculation leads to increased sensitivity and a higher likelihood of ejaculating quickly.
2. Physiological Factors: Increased sensitivity of the glans (the head of the penis) can be a physiological cause. Conditions such as prostatitis or hormonal imbalances may also play a role.
3. Neurological Factors: Some studies suggest that PE may be linked to the nervous system's functioning. An overactive sympathetic nervous system can lead to quicker ejaculation.
4. Hormonal Factors: Imbalances in hormones, particularly testosterone, can influence sexual function and ejaculation timing.
5. Genetic Factors: There may be a hereditary component, as some men report a family history of PE.
Solutions and Management Strategies
1. Behavioral Techniques:
- Start-Stop Technique: This involves stimulating the penis until you feel close to ejaculation, then stopping until the urge decreases. This can help you gain better control over your ejaculation.
- Squeeze Technique: Similar to the start-stop method, this involves squeezing the base of the penis when you feel close to ejaculation to help delay it.
2. Desensitizing Products: Over-the-counter creams or sprays that contain numbing agents can reduce sensitivity and help prolong the duration before ejaculation. However, these should be used cautiously to avoid affecting your partner's sensitivity.
3. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through Kegel exercises can improve control over ejaculation. These exercises involve repeatedly contracting and relaxing the muscles used to stop urination.
4. Counseling and Therapy: If psychological factors are contributing to PE, speaking with a therapist or counselor can help address anxiety, relationship issues, or other emotional factors.
5. Medication: In some cases, doctors may prescribe selective serotonin reuptake inhibitors (SSRIs) or other medications that can help delay ejaculation. Always consult a healthcare provider before starting any medication.
6. Self-Training: Self-awareness and mindfulness during sexual activity can help you recognize the sensations leading to ejaculation. This awareness can help you learn to control your responses better.
Conclusion
It's important to remember that experiencing premature ejaculation is common and can often be addressed with the right strategies. If you find that these methods are not effective or if your symptoms persist, it may be beneficial to consult a healthcare professional who specializes in sexual health. They can provide personalized advice and treatment options tailored to your specific situation. Open communication with your partner about your experiences and concerns can also foster understanding and support as you work through this issue together.
