Hello Dr.
Hsu: Recently, a male friend of mine has experienced involuntary discharge of fluid from his penis at times, and this occurs outside of sexual activity.
The fluid has a smell similar to that of semen.
He recently started a new job that has been more physically demanding than his previous one.
I would like to know if this could be related to his work fatigue and whether he should seek medical evaluation.

The most common is a semen infection, which can include urethritis, seminal vesiculitis, and prostatitis, all of which can lead to increased secretions.
Additionally, if there is a prolonged period without ejaculation, overflow may occur, which at night is referred to as nocturnal emission; however, it can also happen during the day.
Furthermore, during arousal and erection, some fluid may be secreted, and sometimes the amount can be significant, leading one to believe they may be ill.
Fatigue can lower the immune system, which might also cause some physiological symptoms, although it is less common for secretions to appear from the urethra.
