Glans penis
I have a swollen, red area on my glans that seems to be broken skin.
However, my glans hurts whenever I have an erection.
The redness and peeling on the outside of my foreskin have disappeared, but my genital area still hurts and discharges a yellowish fluid.
It also hurts when I urinate, feeling hot and painful.
Could this be a sexually transmitted infection?

Dr. Li Yilun reply Urology
The symptoms are very similar to gonorrhea; it is recommended to see a physician for examination and treatment as soon as possible.
Do not delay!

Understanding pain and discomfort in the glans, particularly when accompanied by symptoms such as redness, swelling, and discharge, can be concerning and may indicate several potential issues. Based on your description, it sounds like you are experiencing significant discomfort, especially during erections and urination, along with the presence of a yellow discharge. Here are some possible causes and considerations:
1. Infection: One of the most common causes of pain and discharge in the genital area is an infection. This could be a sexually transmitted infection (STI) such as gonorrhea or chlamydia, which can cause urethritis (inflammation of the urethra) leading to pain during urination and discharge. Other infections, such as a yeast infection or bacterial infection, can also cause similar symptoms.
2. Balanitis: This is an inflammation of the glans (the head of the penis) that can occur due to poor hygiene, irritants, or infections. Symptoms often include redness, swelling, and pain, especially during erections. If there is a buildup of smegma (a combination of dead skin cells, oils, and moisture), it can lead to irritation and inflammation.
3. Allergic Reaction or Irritation: Sometimes, irritation can be caused by soaps, lotions, or condoms that contain allergens or irritants. This can lead to redness and discomfort in the area.
4. Phimosis: If the foreskin cannot be easily retracted, it can lead to pain and inflammation, particularly during erections. This condition can also contribute to infections and discomfort.
5. Sexually Transmitted Infections (STIs): As mentioned earlier, STIs can present with symptoms like yours. It is crucial to consider recent sexual activity and whether protection was used. If you suspect an STI, it is important to seek medical attention for testing and treatment.
Recommendations:
- Seek Medical Attention: Given the symptoms you are experiencing, it is essential to consult a healthcare professional, preferably a urologist. They can perform a physical examination, take a medical history, and possibly conduct tests (such as urine tests or swabs) to determine the exact cause of your symptoms.
- Avoid Irritants: Until you can see a doctor, try to avoid any potential irritants. This includes harsh soaps, lotions, or any products that may come into contact with the genital area.
- Maintain Hygiene: Keeping the area clean and dry can help prevent further irritation. Gently wash the area with mild soap and water, and ensure it is thoroughly dried.
- Avoid Sexual Activity: Until you have a clear diagnosis, it may be wise to avoid sexual activity to prevent further irritation or potential transmission of any infection.
- Document Symptoms: Keep track of any additional symptoms, such as fever, increased pain, or changes in the discharge, as this information can be helpful for your healthcare provider.
In conclusion, while it is possible that your symptoms could be related to an infection or another medical condition, only a healthcare professional can provide an accurate diagnosis and appropriate treatment. Do not hesitate to seek help, as early intervention can lead to better outcomes and relief from your symptoms.
