Abnormal weight gain and lymphatic tumors in the throat?

Hello, Dr.
Tsai.
I apologize for interrupting you during your busy schedule.
I have a friend who is a female, 157 cm tall, and her weight was originally 39 kg.
However, in the past half month, she suddenly gained 5 kg.
Her daily routine involves waking up around 8:35 AM and going to bed around 1 to 2 AM.
She eats three meals a day (with a larger portion than the average girl), has a habit of drinking beverages (milk tea with two creamers, at least one cup a day), and also has a habit of eating late-night snacks.
She used to not gain weight easily, but now she has suddenly gained weight.
I would like to ask if this situation is normal or abnormal.
Additionally, she has a small growth in her throat (located around the area between her chin and neck).
She mentioned that she has seen a doctor, who said it is a small tumor caused by lymphadenitis that can be removed surgically, but it may recur and can be managed with medication.
I would like to inquire about the possible causes of this condition, why it might recur, and whether it poses any significant health risks.
Thank you, doctor.
I greatly appreciate your help.

1.
The causes of weight gain can be physiological factors: binge eating, irregular lifestyle and dietary habits (excessive intake of carbohydrates or fats, insufficient exercise) and pathological factors: large tumors, abdominal ascites, edema.

2.
Lymphadenopathy: Benign: soft, sudden onset within a few days, less than one centimeter, painful, often due to viral or bacterial lymphadenitis, and may also be due to cysts.
Malignant: hard, progressively enlarging over several months, painless, fixed and non-mobile, often due to metastatic tumors in the cervical lymph nodes or pulmonary tuberculosis.

More Info

Dear Patient,
Thank you for reaching out with your concerns regarding your friend's sudden weight gain and the presence of a lymph node tumor. These issues can be quite distressing, and it's important to understand the potential underlying causes and implications.

Unexplained Weight Gain
Sudden weight gain, especially when it occurs over a short period, can be attributed to various factors. In your friend's case, gaining 5 kilograms in half a month is significant and warrants attention. Here are some possible explanations:
1. Dietary Changes: You mentioned that your friend has been eating more than usual and has a habit of consuming high-calorie beverages like milk tea. Increased caloric intake can lead to weight gain, especially if it exceeds the number of calories burned through daily activities.

2. Fluid Retention: Weight gain can also result from fluid retention, which can be caused by various factors, including hormonal changes, high sodium intake, or certain medications. If your friend has been consuming more salty foods or drinks, this could contribute to temporary weight gain.

3. Lifestyle Factors: Changes in physical activity levels can also impact weight. If your friend has become less active or has altered her routine, this could lead to weight gain.

4. Medical Conditions: Certain medical conditions, such as hypothyroidism, polycystic ovary syndrome (PCOS), or other hormonal imbalances, can cause unexplained weight gain. It would be advisable for her to consult a healthcare provider for a thorough evaluation, including blood tests to check thyroid function and other hormonal levels.

Lymph Node Tumors
Regarding the small lump your friend has noticed in the area between her jaw and neck, which has been diagnosed as an inflamed lymph node, there are several points to consider:
1. Lymphadenopathy: Swollen lymph nodes can occur due to various reasons, including infections (viral or bacterial), inflammatory conditions, or malignancies. In many cases, lymph nodes swell in response to infections, and they may remain enlarged for some time even after the infection has resolved.

2. Chronic Inflammation: If the lymph node is persistently swollen, it could indicate chronic inflammation or an ongoing infection. Conditions such as chronic tonsillitis or dental issues can lead to prolonged lymph node enlargement.

3. Surgical Intervention: The recommendation for surgical removal of the lymph node may be appropriate if it is causing discomfort or if there is a concern about the nature of the swelling. However, it is essential to understand that lymph nodes can remain reactive and may swell again after removal, especially if the underlying cause is not addressed.

4. Monitoring and Follow-Up: It is crucial for your friend to have regular follow-ups with her healthcare provider to monitor the lymph node and assess any changes. If the lymph node continues to grow or if new symptoms develop, further investigation, including imaging studies or a biopsy, may be necessary to rule out more serious conditions.

Conclusion
In summary, your friend's sudden weight gain and the presence of a lymph node tumor should be evaluated by a healthcare professional. A comprehensive assessment, including a detailed medical history, physical examination, and possibly laboratory tests, will help determine the underlying causes and appropriate management strategies. Encourage her to seek medical advice promptly to address these concerns effectively.

Thank you for your inquiry, and I hope this information helps clarify your friend's situation. Please feel free to reach out if you have any further questions or need additional assistance.

Best regards.
