Varicocele
Last October, I experienced pain in the left side of my scrotum, so I went to the hospital for an ultrasound examination.
It was confirmed that I have a varicocele.
However, the doctor did not provide any treatment.
Occasionally, I still feel pain in my left testicle.
I would like to ask whether it is better to wear boxers or briefs.
Boxers allow the testicles to hang down for better heat dissipation but may impair venous return, while briefs do the opposite.
How should I choose? Thank you.

It is indeed difficult to choose; in fact, both triangular and square shapes are the same.
Varicocele primarily results from poor venous return, which is especially noticeable when standing.
Prolonged standing can sometimes lead to pain due to venous swelling.
However, pain in the scrotum can also be caused by epididymitis, and the causes of the two conditions are different.
The choice between triangular or square shapes is not related to this issue.
I recommend consulting a urologist for further discussion.

When it comes to choosing the right underwear for managing varicocele, the decision between boxers and briefs can indeed have implications for comfort and potentially for the condition itself. Varicocele, which is characterized by enlarged veins within the scrotum, can lead to discomfort and pain, particularly if the veins are not functioning properly.
Understanding Varicocele
Varicocele is often compared to varicose veins in the legs. It occurs when the valves in the veins that carry blood away from the testicles fail to function properly, leading to blood pooling and vein enlargement. This can result in a variety of symptoms, including pain, swelling, and even fertility issues. The discomfort can be exacerbated by prolonged standing or physical activity, and it is often more noticeable in warm conditions.
Boxers vs. Briefs
1. Boxers:
- Pros: Boxers provide a looser fit, which allows for better airflow and can help keep the scrotum cooler. This is beneficial because cooler temperatures are generally better for sperm production and overall testicular health. The loose fit may also reduce pressure on the veins, potentially alleviating some discomfort.
- Cons: However, the lack of support can lead to increased movement of the testicles, which might cause discomfort for some individuals, especially if they are already experiencing pain due to varicocele.
2. Briefs:
- Pros: Briefs offer more support and can help keep the testicles in a more stable position. This can reduce movement and may help alleviate some discomfort associated with varicocele. The snug fit can also provide a sense of security, which some men find comforting.
- Cons: On the downside, briefs can trap heat, which may not be ideal for testicular health. Increased warmth can negatively impact sperm production, so it’s important to find a balance.
Recommendations
Given your situation with varicocele and the occasional pain in your left testicle, here are some recommendations:
- Consider Supportive Underwear: If you find that your pain is exacerbated by movement, you might want to try supportive briefs or even specialized men's underwear designed for support. Look for options that provide a snug fit without being overly tight, as excessive pressure can also be counterproductive.
- Monitor Temperature: Regardless of the type of underwear you choose, be mindful of temperature. If you opt for briefs, consider materials that wick moisture and allow for breathability to help keep the area cool.
- Listen to Your Body: Ultimately, the best choice is the one that feels most comfortable for you. If you notice that one type of underwear exacerbates your symptoms, it may be worth switching to the other option.
- Consult Your Doctor: Since you have a diagnosed varicocele and are experiencing pain, it’s advisable to keep your healthcare provider informed about your symptoms. They may recommend further evaluation or treatment options if your discomfort persists.
Conclusion
In summary, both boxers and briefs have their pros and cons when it comes to managing varicocele. The choice largely depends on your personal comfort and how each type of underwear affects your symptoms. It’s essential to prioritize support while also considering temperature regulation to promote testicular health. If pain continues or worsens, seeking medical advice is crucial to ensure that your condition is managed appropriately.
