I don't know what disease it is?
Hello Dr.
Chang: My penis has been itching at the base since the day after I had protected sexual intercourse with a stranger.
The itching is more pronounced when I walk, and it is less severe when I am sitting.
When I check the itchy area, it appears slightly red, but there is no pain, just itching.
I have visited the dermatology department at Taichung Veterans General Hospital twice, and the doctors said that my skin looks normal and they could not determine the cause (the doctors also confirmed it is not pubic lice, and I have not noticed any parasites myself).
I initially thought it was pubic lice, and shaving the hair did provide some relief, but the itching returned once the hair grew back.
This symptom has persisted for nearly five months, and I have not scratched it.
1.
Based on the symptoms described, what could be the possible cause?
2.
Is it possible that this could be a symptom of syphilis or another sexually transmitted infection?

Dr. Zhang Zhibo reply Dermatology
Hello: The symptoms you described make it difficult to determine if it is a lice issue.
If it is pubic lice, typically shaving the area and applying medication for 1-2 days should suffice, as pubic lice lay eggs and can also cause dermatitis symptoms (eczema).
It is important to treat the eczema as well.
If the condition does not improve after a month, a skin biopsy should be considered, especially if there are signs of a rash or other obvious skin lesions.
If there is no rash and only itching, then antihistamines can be used to relieve the itching.
Additionally, syphilis or other sexually transmitted infections do not typically present with itching as the sole symptom.
I recommend consulting a dermatologist for further evaluation.
Wishing you a speedy recovery.

Persistent itching in the genital area can be a distressing and uncomfortable experience, and it’s important to approach this issue with care and consideration. Based on your description, it seems that you have been experiencing itching at the base of your penis for about five months following a sexual encounter. The fact that the itching is more pronounced when walking and less so when sitting, along with the presence of slight redness, suggests that there may be an underlying dermatological or irritative cause.
1. Possible Causes:
- Irritation or Allergic Reaction: The use of condoms, lubricants, or even personal hygiene products can sometimes cause irritation or allergic reactions, leading to itching and redness. If you used a new type of condom or lubricant during the encounter, this could be a potential cause.
- Folliculitis: This is an inflammation of the hair follicles that can occur after shaving or waxing. If you recently shaved the area, it might lead to irritation and itching as the hair begins to grow back.
- Dermatitis: Contact dermatitis, which can be caused by soaps, detergents, or other substances that come into contact with the skin, might also be a factor.
- Fungal Infections: Conditions like jock itch (tinea cruris) can cause itching and redness in the groin area. This is more common in warm, moist environments.
- Bacterial Infections: Sometimes, bacterial infections can lead to localized itching and irritation.
- Sexually Transmitted Infections (STIs): While you mentioned that the doctors ruled out lice and that you have not noticed any bugs, it is still important to consider STIs. Conditions such as herpes, gonorrhea, or chlamydia can sometimes present with itching and irritation, although they often have additional symptoms.
2. Concerns Regarding STIs:
- While the symptoms you describe do not definitively indicate an STI, it is crucial to rule out these possibilities, especially since you had a sexual encounter. Symptoms of STIs can vary widely, and some infections can be asymptomatic. If you have not been tested for STIs since the encounter, it would be advisable to do so. Testing can provide peace of mind and help identify any underlying issues that may require treatment.
Given that you have already consulted with dermatologists and they found no visible abnormalities, it may be beneficial to seek a second opinion or consult a urologist or an infectious disease specialist. They can provide a more comprehensive evaluation and may suggest specific tests for STIs or other conditions that may not have been considered previously.
In the meantime, here are some general recommendations:
- Avoid Irritants: Try to identify and avoid any potential irritants, such as new soaps, detergents, or personal care products.
- Keep the Area Dry: Moisture can exacerbate itching, so keeping the area dry and clean is essential. Wearing loose-fitting underwear can also help.
- Over-the-Counter Treatments: If the itching is bothersome, over-the-counter hydrocortisone cream may help reduce inflammation and itching, but it’s best to consult with a healthcare provider before starting any treatment.
In conclusion, persistent itching in the genital area can stem from various causes, including irritation, infections, or STIs. It is essential to consult with a healthcare professional who can provide a thorough examination and appropriate testing to determine the cause of your symptoms. Taking proactive steps to address your concerns will help you find relief and ensure your overall health and well-being.
