Strange phenomena after erection and ejaculation?

Hello, Dr.
Yan.
I have been troubled by a peculiar condition for almost two years.
I would greatly appreciate your insights.
Every time I experience an erection, I feel a bloating sensation in my lower abdomen, as if air is being pumped in.
This sensation becomes more pronounced during ejaculation.
After ejaculation, my entire lower abdomen feels hard and distended, as if it is filled with gas.
Additionally, ejaculation has become quite laborious, unlike before when it was effortless.
In certain positions, it feels as though the semen is just flowing out, as if it is stuck and unable to be expelled.

Moreover, for the past year, I have noticed that upon waking up every morning, my bladder feels as if it is about to burst, and I have an excessive amount of urine.
This was not the case before, and my normal urination frequency has also increased.
I am unsure what the underlying issue could be.
I have previously undergone gastroscopy and colonoscopy, and both my stool and urine tests have returned normal results.
I am truly at a loss for the cause of these symptoms.
I kindly ask for your assistance, and I am very grateful for your help.

More Info

It sounds like you are experiencing a complex set of symptoms that can be quite distressing. Based on your description, it appears that you are facing discomfort in your lower abdomen during and after erections and ejaculation, as well as changes in urinary frequency and urgency. Let's break down your symptoms and explore potential causes and recommendations.

Symptoms Breakdown
1. Abdominal Distension During Erection and Ejaculation: The sensation of your lower abdomen feeling inflated or distended during an erection and becoming more pronounced during ejaculation could be related to several factors. This could include increased intra-abdominal pressure during sexual arousal and ejaculation, which may lead to discomfort. It’s also possible that there is a muscular or structural issue in the pelvic region that is contributing to this sensation.

2. Difficulty with Ejaculation: The feeling of difficulty or strain during ejaculation, along with the sensation of the semen being "stuck," could indicate a problem with the prostate or seminal vesicles. Conditions such as prostatitis (inflammation of the prostate) or seminal vesiculitis (inflammation of the seminal vesicles) can lead to discomfort and changes in ejaculation.

3. Increased Urinary Frequency and Urgency: The change in your urinary habits, particularly the feeling of urgency upon waking and increased frequency throughout the day, could be indicative of a urinary tract issue or a prostate condition. Conditions such as benign prostatic hyperplasia (BPH) or prostatitis can lead to increased urinary frequency and urgency, especially in men over the age of 40.

Possible Causes
- Prostatitis: This condition can cause pelvic pain, discomfort during ejaculation, and urinary symptoms. It can be acute or chronic and may require specific treatment.

- Benign Prostatic Hyperplasia (BPH): This is a common condition in older men where the prostate enlarges, leading to urinary symptoms such as increased frequency and urgency.

- Pelvic Floor Dysfunction: Issues with the muscles of the pelvic floor can lead to discomfort during erections and ejaculation, as well as urinary issues.

- Psychological Factors: Anxiety or stress related to sexual performance can also manifest as physical symptoms, including discomfort during sexual activity.

Recommendations
1. Consult a Urologist: Given the complexity of your symptoms, it is crucial to consult a urologist who specializes in male reproductive health. They can perform a thorough examination, including a digital rectal exam, and may recommend further tests such as a prostate-specific antigen (PSA) test, urinalysis, or imaging studies.

2. Pelvic Floor Therapy: If pelvic floor dysfunction is suspected, a referral to a physical therapist who specializes in pelvic health may be beneficial. They can provide exercises and techniques to help alleviate symptoms.

3. Lifestyle Modifications: Consider keeping a diary of your symptoms, including when they occur and any potential triggers. This information can be helpful for your healthcare provider. Additionally, staying hydrated and maintaining a healthy diet may help with urinary symptoms.

4. Follow-Up Testing: Since you have already undergone endoscopic examinations (gastroscopy and colonoscopy) that were normal, it may be worthwhile to explore further urological evaluations, including a cystoscopy if urinary symptoms persist.

5. Mental Health Support: If anxiety or stress is a contributing factor, speaking with a mental health professional may help you manage these feelings and improve your overall well-being.

Conclusion
Your symptoms are certainly concerning, and it is important to address them with a healthcare professional who can provide a comprehensive evaluation and appropriate treatment. Do not hesitate to seek help, as many conditions can be effectively managed with the right approach. Remember, you are not alone in this, and there are resources available to help you navigate these challenges.
