Pain and discomfort in the left testicle, along with abdominal pressure and pain?
Hello Doctor, this issue has persisted since high school and seems to have worsened recently.
I now experience soreness when I touch the bottom of my left testicle and lift it upwards.
Additionally, when I palpate above the testicle, the cord feels thickened, and it is also tender to the touch.
What could be the cause of this? Thank you in advance for your response.
Wishing you a good day.

Left testicle pain accompanied by abdominal discomfort can be a complex issue, and it is essential to consider various potential causes. Given your history of symptoms that have persisted since high school and have recently worsened, it is crucial to approach this with a comprehensive understanding of the anatomy and possible medical conditions involved.
First, let's discuss the anatomy. The testicles are located in the scrotum, and they are connected to the abdomen via the spermatic cord, which contains blood vessels, nerves, and the vas deferens. Pain in the left testicle can often be related to issues within the testicle itself or conditions affecting the surrounding structures.
One common cause of testicular pain is epididymitis, which is the inflammation of the epididymis, the tube that stores and carries sperm. This condition can be caused by infections, including sexually transmitted infections, or non-infectious causes such as trauma or a reaction to certain medications. Symptoms typically include swelling, redness, and pain in the affected testicle.
Another potential cause is testicular torsion, a medical emergency where the spermatic cord twists, cutting off blood supply to the testicle. This condition usually presents with sudden, severe pain and swelling and requires immediate medical attention to prevent permanent damage.
Varicocele, which is an enlargement of the veins within the scrotum, can also lead to a feeling of heaviness or discomfort in the testicle. It is often described as a "bag of worms" when palpated and can cause dull, aching pain, especially after prolonged standing or physical activity.
Additionally, hernia could be a contributing factor, particularly if there is a bulge in the groin area. An inguinal hernia occurs when tissue protrudes through a weak spot in the abdominal muscles, which can lead to discomfort in the lower abdomen and groin, sometimes radiating to the testicle.
Referred pain from abdominal conditions such as kidney stones or gastrointestinal issues can also manifest as testicular pain. Conditions like gastroesophageal reflux disease (GERD) or irritable bowel syndrome (IBS) can lead to discomfort that may be perceived in the testicular area due to shared nerve pathways.
Given your description of the pain being more pronounced when you touch the area and the sensation of thickened cords, it would be prudent to consider a few diagnostic steps. A thorough physical examination by a healthcare provider is essential, as they can assess for any swelling, tenderness, or abnormalities in the testicle and surrounding structures.
Imaging studies, such as an ultrasound, can help visualize the testicles and spermatic cord, allowing for the identification of conditions like epididymitis, torsion, or varicocele. Blood tests may also be warranted to check for signs of infection or other underlying conditions.
In conclusion, left testicle pain accompanied by abdominal discomfort can stem from various causes, including infections, anatomical abnormalities, or referred pain from other areas. It is crucial to seek medical evaluation to determine the exact cause of your symptoms and to receive appropriate treatment. Early intervention can prevent complications and improve your quality of life. If you experience sudden, severe pain, swelling, or any other alarming symptoms, do not hesitate to seek emergency medical care.
