Irregular menstrual flow?

Dear Dr.
Huang,
1.
My menstrual flow is usually heavier from the first to the third day, but it suddenly decreases on the fourth day to the point where I only need to use a panty liner, and then it increases again.
My menstrual cycle lasts for 7 days.
Is this situation normal? This time in June, my period came and after 2 days, the flow became very light and lasted for about 3 days.
Could this be related to my increased consumption of cold beverages?
2.
Two months ago, after my period, I started feeling some pain near the vaginal opening when urinating, but I don't experience pain at other times or when sitting.
Could this be related to my habit of excessively wiping my bottom every time I change my pad during my period? I have also touched the area near my anus but did not find any wounds.
I kindly ask for your medical advice, thank you.
Wishing you good health.

Xiao Yan, 20~29 year old female. Ask Date: 2008/07/12

Please adjust your dietary habits to see if there is any improvement.

Irregular menstrual flow and related discomforts can be concerning for many women, and it’s important to understand the potential causes and implications of these symptoms. Based on your description, let's address your questions in detail.

1. Menstrual Flow Variability: It is not uncommon for menstrual flow to vary throughout the duration of a period. Many women experience heavier bleeding in the first few days, which gradually tapers off. The pattern you describe—heavy flow on days one to three, followed by a sudden decrease on day four, and then an increase again—can be considered normal for some women. However, significant changes in your menstrual cycle, such as a sudden shift to a very light flow or unexpected bleeding, should be evaluated by a healthcare provider.

Regarding your observation of a sudden light flow after two days of menstruation, it is unlikely that drinking cold beverages directly affects your menstrual flow. However, lifestyle factors, stress, hormonal changes, and even dietary habits can influence your cycle. If this pattern continues or if you experience additional symptoms, such as severe pain or significant changes in your cycle, it would be wise to consult with a gynecologist for further evaluation.

2. Pain Near the Vaginal Opening: The discomfort you are experiencing near the vaginal opening after your menstrual period could be related to several factors. It is possible that irritation from frequent wiping or changing sanitary products could lead to localized discomfort. The vaginal area is sensitive, and excessive friction or irritation can cause pain. Additionally, hormonal changes during your menstrual cycle can lead to increased sensitivity in the vaginal area.

If the pain persists or worsens, or if you notice any unusual discharge, it would be advisable to seek medical attention. A healthcare provider can perform a physical examination to rule out any infections, skin conditions, or other underlying issues that may be contributing to your discomfort.

In summary, while some variability in menstrual flow can be normal, significant changes or persistent discomfort should not be overlooked. It is always best to consult with a healthcare professional who can provide personalized advice based on a thorough assessment of your symptoms and medical history. Regular gynecological check-ups are also important for maintaining reproductive health and addressing any concerns that may arise.
If you have any further questions or if your symptoms change, please do not hesitate to reach out to your healthcare provider. Your health and comfort are paramount, and getting the right advice is crucial for your well-being.
