the Connection Between Urological Issues and Sexual Performance - Urology

Is there a correlation?

Dear Doctor Hsu,
I have mitral valve prolapse and I'm wondering if it is related to my strong sexual desire but very short erection duration (less than a minute).
During intercourse, I often lose my erection halfway and need to stimulate myself for a long time (even though I still have sexual desire) to continue.
I do not have any bad habits, my lifestyle is normal, and I am certain that it is not a psychological issue.
Why is this happening? A friend suggested using a topical treatment like Viagra.
Is this advisable? Thank you, Doctor.

Zhuang Xiansheng, 20~29 year old female. Ask Date: 2008/07/16

Dr. Xu Weikai reply Urology

Mr.
Chuang: The pressure in the penis is approximately two-thirds of the blood pressure, which indicates that normal heart function is necessary for a perfect erection.
Regarding your mitral valve prolapse, I'm unsure of the severity; if it's severe, it could potentially have an impact.
You mentioned "Beishodeng"? I'm not familiar with it and cannot comment on its ingredients.
Additionally, sometimes erectile dysfunction may occur due to the presence of an arteriovenous fistula, where blood flows directly back into the veins without passing through the penis, which can affect both the duration and hardness of an erection.
If there are any unclear points, feel free to ask again, and we can further discuss and research together.
Sincerely, Urology Department, Taitung Hospital, Ministry of Health and Welfare, Dr.
Hsu Wei-Kai.

Understanding the connection between urological issues and sexual performance is crucial for addressing concerns related to erectile dysfunction, pain during sexual activity, and overall sexual health. Based on the information provided, it seems you are experiencing erectile difficulties that are not linked to psychological factors, and you have a strong sexual desire. This situation can be frustrating and may lead to feelings of inadequacy or anxiety, but it's essential to approach it with a clear understanding of potential underlying causes.

Erectile dysfunction (ED) can stem from various physical issues, including vascular problems, hormonal imbalances, neurological conditions, and anatomical abnormalities. In your case, you mentioned having a condition known as "二間瓣脫垂" (which translates to "two-leaf valve prolapse"), though it is not entirely clear how this condition relates to your erectile function. Prolapse conditions can sometimes affect pelvic floor muscles and overall pelvic health, which may indirectly influence sexual performance.

When it comes to the duration of erections, several factors can play a role. The physiological process of achieving and maintaining an erection involves a complex interplay of blood flow, nerve signaling, and muscle relaxation. If any part of this process is disrupted, it can lead to difficulties in maintaining an erection. For instance, issues with blood flow due to vascular health, nerve damage, or even hormonal levels (such as testosterone) can significantly impact erectile function.

You mentioned that your erections tend to be short-lived, often requiring manual stimulation to maintain them. This could indicate that while you have the desire and initial arousal, there may be a physiological barrier preventing you from sustaining the erection. It's also worth noting that factors such as fatigue, stress, or even certain medications can contribute to these issues.

Regarding your friend's suggestion to use a topical treatment like "倍碩登" (which is likely a brand of topical erectile dysfunction medication), it's essential to approach this with caution. While some topical treatments can enhance blood flow to the area, they are not a substitute for addressing the underlying causes of erectile dysfunction. Moreover, using such products without medical supervision can lead to adverse effects or complications.

It is highly recommended that you consult a urologist or a healthcare professional specializing in sexual health. They can perform a thorough evaluation, including a physical examination, blood tests to check hormone levels, and possibly imaging studies to assess blood flow and anatomical structures. This comprehensive approach will help identify any underlying conditions that may be contributing to your symptoms.

In addition to medical evaluation, lifestyle factors should also be considered. Maintaining a healthy diet, regular exercise, and managing stress can all positively impact sexual health. If anxiety or performance pressure is a concern, discussing these feelings with a therapist or counselor may also be beneficial.

In summary, while your strong sexual desire is a positive aspect, the challenges you face with erectile function warrant further investigation. A healthcare professional can provide tailored advice and treatment options based on your specific situation, ensuring that you receive the most effective care for your sexual health concerns.
