Ejaculation
Hello, doctor.
A few days ago, I experienced something during masturbation.
When I ejaculated, I noticed that during the first one or two contractions, no semen was expelled.
It was only later that semen came out, and the amount seemed to be quite small.
However, I did not experience any other discomfort during the process.
This has happened twice in the past couple of months, which has caused me significant concern.
I'm wondering if I might have some kind of medical issue.
Could you please help me with this? Thank you.
JJ, 20~29 year old female. Ask Date: 2008/07/20
Dr. Xu Weikai reply Urology
Hello, this situation may be due to excessive masturbation, which can lead to a reduced volume of semen in the seminal vesicles, resulting in insufficient ejaculation.
Additionally, long-term masturbation may decrease stimulation, causing a gradual decline in ejaculate volume.
Changing your masturbation technique might help increase libido, which in turn could enhance ejaculate volume.
Furthermore, insufficient ejaculate volume may reflect an overall poor health condition, suggesting the need for lifestyle adjustments, such as avoiding late nights and maintaining regular meal times.
If you have any further questions or need more detailed information, feel free to ask for further discussion.
Sincerely,
Dr.

Delayed ejaculation is a condition where a man has difficulty achieving ejaculation during sexual activity, including masturbation. This can be a source of concern and confusion, especially if it is a new experience. There are several potential causes and factors that can contribute to this condition, and understanding them can help alleviate some of the anxiety surrounding it.
Firstly, it is important to note that occasional instances of delayed ejaculation can be normal and may not necessarily indicate a serious underlying issue. Factors such as stress, anxiety, fatigue, or changes in routine can temporarily affect sexual function. If you have been experiencing increased stress or anxiety in your life, this could be a contributing factor. Psychological factors are often significant in sexual health, and they can impact not just the ability to ejaculate but also libido and overall sexual satisfaction.
Secondly, physical health plays a crucial role in sexual function. Conditions such as diabetes, neurological disorders, hormonal imbalances, or certain medications can affect ejaculation. If you are taking any medications, particularly those for mental health conditions or blood pressure, they may have side effects that impact sexual function. It would be beneficial to review any medications you are currently taking with your healthcare provider to determine if they could be contributing to your symptoms.
Additionally, lifestyle factors such as alcohol consumption, smoking, and drug use can also affect sexual performance. If you have made any recent changes in these areas, it might be worth considering their potential impact on your sexual health.
In your case, since you mentioned that this has happened a couple of times over the past month and that you did not experience any discomfort, it may be a temporary issue. However, if this continues to be a concern or if you notice any additional symptoms, it would be advisable to consult with a healthcare professional. They can conduct a thorough evaluation, which may include a physical examination and possibly some tests to rule out any underlying medical conditions.
Moreover, communication with your partner about your experiences can also be beneficial. Open discussions about sexual health can help reduce anxiety and improve intimacy, which may, in turn, enhance sexual function.
In summary, while occasional delayed ejaculation can be a normal occurrence, persistent issues warrant further investigation. Factors such as psychological stress, physical health, medication side effects, and lifestyle choices can all play a role. If your symptoms persist or worsen, seeking advice from a healthcare provider specializing in sexual health can provide you with the support and information you need to address your concerns effectively. Remember, sexual health is an important aspect of overall well-being, and there are resources available to help you navigate these challenges.
