Finger stiffness and pain inquiry?

Hello Doctor, I would like to ask about my fingers.
Initially, when I wake up in the morning, they feel stiff.
After two weeks, the stiffness turned into pain, particularly around the nails and the first joint of my fingers.
However, once I move them, the discomfort improves significantly.
This happens every morning, and sometimes it affects both hands or just a few fingers, with my left hand being more frequently affected.
Throughout the day, I also experience occasional pain or slight discomfort when bending my fingers, and pressing on the joints can cause mild pain that resolves quickly.
After four weeks, I started to feel a bit of nerve pain or tingling sensation in the flesh below my thumb, but it lasts only briefly.
I have consulted both orthopedic and rheumatology specialists, and after blood tests, the doctors said everything is normal.
I found online that it could possibly be De Quervain's tenosynovitis or "mouse hand." I would like to know if my symptoms align with these conditions, or if you have any recommendations on what specialty I should consult? I'm quite worried.
Thank you.

Hello, sometimes the sensation of stiffness in the hands may be related to cervical spine issues.
Carpal tunnel syndrome commonly presents with numbness in the fingers or palm, and may occasionally extend to the forearm, but it is rare to manifest as localized joint tenderness.
It is recommended to follow up with the original treatment facility or consult the original physician to see if a referral to another specialty is necessary.
Wishing you peace and health.
Sincerely, Dr.

More Info

Understanding finger stiffness and pain can be quite complex, especially when symptoms like morning stiffness, pain in the joints, and occasional numbness or tingling are involved. Based on your description, it seems you are experiencing a combination of symptoms that could be indicative of several underlying conditions.

Possible Causes
1. Morning Stiffness: The stiffness you experience in the morning is often associated with conditions like arthritis, particularly osteoarthritis or rheumatoid arthritis. In these conditions, inflammation can lead to stiffness, especially after periods of inactivity, such as overnight sleep.
2. Joint Pain: The pain you feel in the joints, particularly around the fingernails and the first joint, could suggest inflammation or irritation in the joint structures. This could be due to repetitive strain injuries, which are common in individuals who use their hands extensively, such as those who work on computers or engage in sports.

3. Nerve Compression: The tingling or numbness you describe, especially in the thumb area, could be related to nerve compression syndromes, such as carpal tunnel syndrome. This condition occurs when the median nerve is compressed as it travels through the wrist, leading to symptoms in the hand.

4. Tendinitis: Given your history of using your hands extensively, tendinitis could also be a factor. This condition involves inflammation of the tendons, which can lead to pain and stiffness, particularly during movement.

5. Other Conditions: Conditions like trigger finger, De Quervain's tenosynovitis, or even systemic conditions like lupus or fibromyalgia could also cause similar symptoms. However, since your blood tests came back normal, systemic inflammatory conditions may be less likely.

Recommendations
1. Consult a Specialist: Since you have already seen orthopedic and rheumatology specialists, it may be beneficial to consult a neurologist, especially if you continue to experience numbness or tingling. They can perform nerve conduction studies to assess for conditions like carpal tunnel syndrome or other neuropathies.

2. Physical Therapy: Engaging in physical therapy can help improve flexibility and strength in your fingers and hands. A physical therapist can provide exercises tailored to your specific symptoms and needs.

3. Ergonomic Adjustments: If you suspect that your symptoms are related to repetitive strain from activities like typing or using a mouse, consider making ergonomic adjustments. This could include using a wrist rest, adjusting your chair height, or ensuring your workstation is set up to minimize strain on your hands.

4. Activity Modification: Reducing activities that exacerbate your symptoms, especially those involving repetitive hand motions, can help alleviate pain. Incorporating regular breaks during activities can also be beneficial.

5. Pain Management: Over-the-counter anti-inflammatory medications, such as ibuprofen or naproxen, can help manage pain and inflammation. However, consult with your healthcare provider before starting any new medication.

6. Warm-Up Exercises: Gentle stretching and warm-up exercises for your hands and fingers before engaging in activities can help reduce stiffness and pain.

Conclusion
Your symptoms of morning stiffness, joint pain, and occasional numbness warrant further investigation to determine the underlying cause. While conditions like arthritis and nerve compression syndromes are possibilities, a thorough evaluation by a neurologist and possibly a physical therapist could provide more clarity and effective management strategies. Remember to listen to your body and seek help when needed, as early intervention can often lead to better outcomes.
