Ureteroscopy with Stone Management (URS-SM) - Hematuria Issues?

Dear Dr.
Chang,
I have some questions regarding urinary tract stones and the ureteroscopy with stone manipulation (URS-SM) procedure.

1.
How many days is it normal for hematuria to persist after the surgery? Is it normal to feel a dull, bloated sensation in the lower abdomen?
2.
If the color of the hematuria does not lighten day by day and remains as dark as it was immediately after the surgery, is that considered normal? (It has been 4 days since July 21.)
3.
Are the above conditions normal? Could the position of the internal stent be misaligned and causing pressure? Is it possible for the gradual loss of blood to be excessive? Could this have an impact on my health? Should I consider an earlier follow-up appointment? (My next appointment is on July 29.)
I apologize for bothering you during your busy schedule and sincerely appreciate your answers and explanations.
Thank you very much, Dr.
Chang!
Sincerely,
Mr.
Yeh

Hello: 1.
If the double J stent is removed within three days, the presence of a retained double J catheter may cause mild hematuria, which typically resolves within seven days.
2.
If it has not been more than seven days and the double J stent remains in place, hematuria may persist for a longer duration.
3.
It is not expected to have any adverse effects on the body; however, it is advisable to return for a follow-up appointment, which usually occurs within seven days post-surgery.
If you have any concerns, you may visit your treating physician's clinic.

After undergoing ureteroscopic stone surgery (URS-SM), it's common for patients to have concerns regarding postoperative symptoms, particularly blood in the urine (hematuria) and abdominal discomfort. Let's address your questions one by one.

1. Duration of Hematuria: It's typical for patients to experience some degree of hematuria following URS-SM. Generally, if a double-J stent (ureteral stent) is placed, mild hematuria can persist for a few days to a week post-surgery. In your case, since it has been four days since the procedure, some blood in the urine can still be considered normal. However, if the bleeding is significant or worsening, it is essential to consult your physician.

2. Color of Blood in Urine: The color of the urine can vary significantly after surgery. Initially, it may be bright red or dark red, but over time, it should gradually lighten. If you notice that the color remains consistently dark and does not show signs of improvement, this could be a cause for concern. Persistent dark red urine may indicate ongoing bleeding or other complications, and it would be prudent to seek medical advice sooner than your scheduled follow-up.

3. Stent Position and Blood Loss: The discomfort you are experiencing in your lower abdomen could be related to the presence of the stent, which can cause irritation. If the stent is positioned incorrectly, it might lead to increased pain or bleeding. While minor blood loss is usually not harmful, significant or continuous bleeding could lead to complications, including anemia or the need for further intervention. If you are experiencing a steady loss of blood, it is advisable to return to your healthcare provider for an evaluation.

In summary, while some postoperative symptoms like hematuria and abdominal discomfort can be normal after URS-SM, it is crucial to monitor the severity and duration of these symptoms. If you notice any worsening of your condition, such as increased pain, persistent dark urine, or significant blood loss, do not hesitate to seek medical attention before your scheduled follow-up appointment. Your health and well-being are paramount, and timely intervention can prevent potential complications.

Additionally, maintaining adequate hydration and following your doctor's postoperative care instructions can help facilitate recovery and reduce the risk of further complications. If you have any further concerns or questions, please feel free to reach out to your healthcare provider for personalized advice and support.
