Regarding upper abdominal pain?

I would like to ask about the severe pain I frequently experience in my upper right abdomen.
Sometimes it occurs in the morning when I wake up, and other times it happens at night when I'm about to sleep.
This has been going on for quite some time.
I previously thought it was a gastrointestinal issue, but the medication prescribed by my doctor hasn't helped.
I would like to know what diseases could cause this type of pain.

Hei Lian Xue, 20~29 year old female. Ask Date: 2008/08/22

Dr. Lin Minghui reply Gastroenterology and Hepatology

Hello, there are many causes of upper abdominal pain, including gastrointestinal issues, inflammation, ulcers, erosion, or functional reasons.
Additionally, liver, gallbladder, and pancreatic diseases may also be involved, as well as referred pain from other organs.
If the pain persists, it is advisable to seek medical evaluation.
Wishing you good health.
Sincerely, Dr.
Lin Ming-Hui, Taoyuan Hospital, Department of Health.

More Info

Upper right abdominal pain can be a complex symptom with various potential causes, particularly given your history of chronic hepatitis B and previous acute hepatitis. Understanding the underlying reasons for this pain is crucial for effective treatment and management.

Possible Causes of Upper Right Abdominal Pain
1. Liver-Related Issues: Given your history of chronic hepatitis B, liver-related complications could be a significant factor. Conditions such as fatty liver disease, liver cirrhosis, or even liver tumors can manifest as pain in the upper right abdomen. Elevated liver enzymes, as you mentioned (GOT and GPT levels between 40-60), indicate ongoing liver inflammation or damage, which could contribute to your symptoms.

2. Gallbladder Problems: The gallbladder, located just beneath the liver, can also be a source of pain. Gallstones or cholecystitis (inflammation of the gallbladder) can cause severe pain, especially after eating fatty meals. If you have a history of gallstones, this could be a likely cause.

3. Kidney Issues: Conditions such as kidney stones or infections can cause referred pain to the upper right abdomen. The pain may radiate from the back to the front, and if you experience any urinary symptoms, this could be a consideration.

4. Gastrointestinal Disorders: While you mentioned that you initially thought it was a gastrointestinal issue, conditions such as peptic ulcers, gastritis, or even pancreatitis could still be relevant. These conditions can cause significant discomfort and may not always respond to standard gastrointestinal medications.

5. Musculoskeletal Pain: Sometimes, pain in the upper abdomen can be due to musculoskeletal issues, such as muscle strain or rib injuries. This type of pain often worsens with movement or certain positions.

6. Referred Pain: Pain from other areas, such as the heart or lungs, can sometimes present as abdominal pain. Although less common, it’s essential to rule out cardiac issues, especially if you have risk factors.

Recommended Actions
Given the persistence and severity of your pain, it is advisable to seek further evaluation at a larger medical facility. A comprehensive assessment may include:
- Imaging Studies: Ultrasound or CT scans can help visualize the liver, gallbladder, and kidneys to identify any structural abnormalities.

- Blood Tests: In addition to liver function tests, checking for markers of inflammation, infection, or other organ function can provide valuable insights.

- Referral to Specialists: Depending on the findings, you may be referred to a gastroenterologist or hepatologist for specialized care.

Treatment Options
Treatment will depend on the underlying cause identified through your evaluations. For liver-related issues, managing your hepatitis B with antiviral medications and regular monitoring is crucial. If gallstones are present, surgical options may be considered. For gastrointestinal issues, dietary modifications and medications to reduce acid or inflammation may be beneficial.

Conclusion
In summary, upper right abdominal pain can stem from various causes, particularly in the context of your medical history. It is essential to pursue further diagnostic evaluations to pinpoint the exact cause and tailor an appropriate treatment plan. Do not hesitate to seek medical attention, especially if the pain worsens or is accompanied by other concerning symptoms. Your health and well-being are paramount, and timely intervention can lead to better outcomes.
