Dull pain in the upper right abdomen (near the lower edge of the right rib cage)?
Hello, Dr.
Tsai: I have been experiencing dull pain in my upper right abdomen for almost two years.
Six months ago, I underwent liver function tests (GOT, GPT) and an ultrasound, both of which returned normal results.
The diagnosis was suspected intercostal neuralgia, but in recent months, the dull pain has become more pronounced and occurs more frequently, although it is still bearable.
I have a gynecological condition and have been intermittently taking antibiotics (cephalosporins and penicillin) for three to four years, so I am concerned whether this could have caused liver damage or if poor posture from sitting at a desk for long periods could also be a contributing factor.

Dr. Cai Junyan reply Gastroenterology and Hepatology
Hello, there are many potential causes for pain in the upper right abdomen, including gallstones, hepatitis, peptic ulcers, or pancreatitis.
Additionally, neuropathic pain, functional gastrointestinal disorders, colorectal pathology, or urinary system disorders could also be possibilities.
Based on your description, liver and gallbladder issues seem less likely, so it would be advisable to consider undergoing an upper gastrointestinal endoscopy and a colonoscopy.
Sincerely, Dr.
Chai Jun-Yan, Gastroenterology Specialist.

More Info
Understanding upper right abdominal pain can be quite complex, as it can stem from a variety of causes, some of which may be benign while others could indicate a more serious underlying condition. Given your history of persistent discomfort in the upper right abdomen for nearly two years, along with normal liver function tests and ultrasound results, it is essential to consider both musculoskeletal and visceral origins of your pain.
Possible Causes of Upper Right Abdominal Pain
1. Musculoskeletal Issues: Your diagnosis of suspected rib nerve pain (intercostal neuralgia) suggests that the pain may be related to nerve irritation or injury in the rib area. This can occur due to poor posture, especially if you spend long hours sitting at a desk. Prolonged sitting can lead to muscle tension and strain in the back and abdominal regions, which may contribute to your discomfort.
2. Visceral Pain: While your liver function tests are normal, it is crucial to consider other potential visceral causes. Conditions affecting the gallbladder, such as gallstones or cholecystitis, can cause pain in the upper right abdomen. Additionally, issues with the pancreas or even the right kidney (such as kidney stones or infections) could also manifest as pain in this area.
3. Gastrointestinal Disorders: Conditions like gastritis, peptic ulcers, or even irritable bowel syndrome (IBS) can lead to discomfort in the upper abdomen. Although these conditions typically present with additional symptoms, they should not be overlooked.
4. Referred Pain: Sometimes, pain in the upper right abdomen may be referred from other areas, such as the spine or even the heart. Given your history of gynecological issues, it’s also worth considering that some pelvic conditions can cause referred pain to the upper abdomen.
Concerns Regarding Antibiotic Use
You mentioned taking antibiotics intermittently for gynecological issues over the past few years. While antibiotics are crucial for treating bacterial infections, long-term use can lead to alterations in gut flora, potentially resulting in gastrointestinal symptoms. However, there is no direct evidence linking long-term antibiotic use to liver disease in the absence of other risk factors.
Recommendations
1. Posture and Ergonomics: Since you suspect that poor sitting posture may be contributing to your pain, consider evaluating your workspace ergonomics. Ensure that your chair and desk are set up to promote good posture, and take regular breaks to stretch and move around.
2. Follow-Up with Healthcare Providers: Given the persistence and change in your symptoms, it would be prudent to follow up with your healthcare provider. They may recommend further imaging studies, such as a CT scan or MRI, to rule out any structural issues that may not have been visible on ultrasound.
3. Pain Management: If your pain is manageable but bothersome, discussing pain management strategies with your doctor could be beneficial. This may include physical therapy, medications, or alternative therapies like acupuncture.
4. Monitor Symptoms: Keep a detailed record of your symptoms, including when they occur, their intensity, and any associated factors (like diet or activity level). This information can be invaluable for your healthcare provider in diagnosing the issue.
5. Consider a Specialist: If your symptoms persist despite initial evaluations, consider seeing a specialist, such as a gastroenterologist or a pain management specialist, who can provide a more focused approach to your symptoms.
In conclusion, while your upper right abdominal pain may be related to musculoskeletal issues or other benign causes, it is essential to remain vigilant and proactive in seeking further evaluation if your symptoms change or worsen. Your health and comfort are paramount, and understanding the underlying causes of your pain is the first step toward effective management.
