Urinary urgency

Doctor, I find myself needing to urinate approximately every hour during my work hours.
I drink about 1600 cc of water throughout the day and around 100 cc after my lunch break.
Just a few minutes after that, I feel a strong urge to urinate.
This has been happening for over a year, and I also feel that my abdomen has become larger.
I'm not sure what the cause might be.
Thank you, doctor.

Mr. Hsieh, 30~39 year old female. Ask Date: 2009/02/14

Dr. Liu Jianting reply Urology

It's normal! If you drink a lot, you'll naturally urinate more.

More Info

Frequent urination, especially during the daytime, can be a concerning symptom for many individuals. In your case, experiencing the urge to urinate every hour during work hours, combined with a relatively moderate fluid intake of around 1600cc, suggests that there may be underlying factors contributing to this urgency. Let's explore some potential causes and solutions.

Possible Causes of Frequent Urination
1. Bladder Capacity and Function: The average adult bladder can hold about 300 to 500cc of urine. If you are feeling the need to urinate frequently, it could indicate that your bladder is not functioning optimally. This could be due to a condition known as "overactive bladder," where the bladder muscles contract involuntarily, leading to a frequent urge to urinate.

2. Fluid Intake and Diuretics: While your fluid intake seems reasonable, certain beverages can act as diuretics, increasing urine production. Caffeinated drinks, alcohol, and even some herbal teas can lead to increased urination. If you consume these types of beverages, they might contribute to your symptoms.

3. Urinary Tract Infections (UTIs): UTIs can cause increased urgency and frequency of urination. They often come with other symptoms such as burning during urination, cloudy urine, or pelvic pain. If you suspect a UTI, it’s essential to consult a healthcare provider for diagnosis and treatment.

4. Prostate Issues (in men): For men, conditions such as benign prostatic hyperplasia (BPH) can lead to urinary frequency and urgency. This is due to the prostate gland pressing against the urethra, making it difficult to fully empty the bladder.

5. Diabetes: Increased urination can be a symptom of diabetes, particularly if accompanied by increased thirst and hunger. If you have other symptoms or a family history of diabetes, it might be worth getting your blood sugar levels checked.

6. Psychological Factors: Stress and anxiety can also lead to increased urinary frequency. If you find that your symptoms worsen during stressful periods, this could be a contributing factor.

7. Hormonal Changes: Hormonal fluctuations, particularly in women, can affect bladder function. Conditions such as menopause can lead to changes in urinary habits.

Solutions and Recommendations
1. Keep a Bladder Diary: Documenting your fluid intake, urination frequency, and any associated symptoms can help identify patterns and triggers. This information can be valuable for your healthcare provider.

2. Limit Diuretics: If you consume caffeine or alcohol, consider reducing your intake to see if your symptoms improve.
3. Scheduled Voiding: Try to establish a regular schedule for urination, even if you don’t feel the urge. This can help train your bladder and reduce urgency.

4. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises like Kegels can improve bladder control and reduce urgency.

5. Consult a Healthcare Provider: Given that you have experienced this issue for over a year, it would be wise to consult a urologist or your primary care physician. They may recommend tests such as a urinalysis, bladder ultrasound, or urodynamic studies to assess bladder function.

6. Address Underlying Conditions: If any underlying medical conditions are identified, such as diabetes or a UTI, appropriate treatment will be necessary to alleviate your symptoms.

7. Lifestyle Modifications: Maintaining a healthy weight and engaging in regular physical activity can also positively impact bladder health.

In conclusion, frequent urination can stem from various causes, ranging from benign to more serious conditions. It’s essential to monitor your symptoms and seek medical advice to determine the underlying cause and appropriate treatment. By taking proactive steps and consulting with a healthcare professional, you can work towards managing your symptoms effectively.
