Questions regarding medications prescribed by physicians?
Hello Dr.
Lin, after our last discussion regarding the erectile issue (#36807), I visited a nearby traditional Chinese medicine clinic.
The physician prescribed a formula consisting of Jiawei Xiaoyao San, Ganlu Yin, and Liuwei Dihuang Wan.
However, my condition remains the same; my genitalia becomes engorged and enlarged, but I am unable to achieve an erection without manual assistance, and it quickly becomes flaccid.
What medication would you recommend instead? Additionally, can I take Jinsuo Gujing Wan and Yougui Wan together? What would be the appropriate dosage in grams for each? Also, I have a strange sensation in the lower back, around the kidney area; it's hard to describe, but it feels empty.
I wonder if this is related to the so-called "lower back and knee weakness"? Thank you for your response.

1.
In principle, Six-Flavor Rehmannia Pill is primarily for maintenance purposes, aimed at preventing further decline in sexual function.
Once degeneration occurs, the recovery speed is slow, and it requires the use of warming herbs such as Gui Fu Ba Wei Wan, You Gui Wan, and Jin Suo Gu Jing Wan for noticeable corrective effects.
2.
Gan Lu Yin is generally used to reduce excessive heat and does not significantly improve sexual function.
3.
Can they be taken together? In principle, yes, but it still depends on your constitution and whether you are taking other medications, as those are warming herbs.
4.
Weakness in the lower back and knees may present as such, and while your description does not seem to align perfectly, there are still signs of kidney deficiency.
It is advisable to consult a trusted physician for treatment, and acupuncture may enhance the effects.

More Info
Dear patient,
Thank you for reaching out with your concerns regarding erectile issues and the treatment you are currently undergoing with Traditional Chinese Medicine (TCM). It's commendable that you are exploring various options to address your condition. Let’s delve into your situation and provide some insights based on TCM principles and practices.
Erectile dysfunction (ED) can stem from various causes, including psychological factors, hormonal imbalances, and physical conditions affecting blood flow. In TCM, the kidneys are often considered the source of vitality and sexual health, and issues related to erectile function are frequently linked to kidney deficiency, particularly "Kidney Yang" deficiency, which can lead to insufficient warmth and energy for sexual function.
The herbal formula you mentioned, 加味逍遙散 (Jia Wei Xiao Yao San), is typically used to soothe the liver and relieve stress, while 甘露飲 (Gan Lu Yin) is known for its ability to nourish the body and clear heat. 六味地黃丸 (Liu Wei Di Huang Wan) is a classic formula for nourishing kidney yin. While these formulas can be beneficial, they may not directly address the specific issue of erectile dysfunction, especially if the underlying cause is related to kidney yang deficiency or blood stagnation.
For your situation, it may be beneficial to consider formulas that specifically target erectile dysfunction. 金鎖固精丸 (Jin Suo Gu Jing Wan) is often used for issues related to premature ejaculation and can help strengthen the kidneys and stabilize essence. 右歸丸 (You Gui Wan) is another formula that tonifies kidney yang and is often recommended for sexual health and vitality.
Regarding your question about taking these two formulas together, it is generally advisable to consult with your TCM practitioner before combining herbal remedies. They can provide personalized guidance based on your specific condition and constitution. As for the dosages, these can vary based on the formulation and your individual needs, so it’s best to follow the recommendations of your practitioner.
The sensation you described in your lower back, which you associate with the kidney area, could indeed be related to kidney deficiency or a form of "腰膝酸軟" (lower back and knee weakness). This symptom is often indicative of a lack of kidney yang or qi, which can contribute to feelings of weakness and discomfort in that area. Strengthening the kidneys through appropriate herbal formulas and possibly acupuncture can be beneficial.
In addition to herbal treatments, lifestyle modifications can also play a crucial role in managing erectile dysfunction. Regular exercise, a balanced diet, stress management techniques, and adequate sleep are all essential components of overall health and can significantly impact sexual function.
Lastly, I encourage you to maintain open communication with your TCM practitioner. They can adjust your treatment plan based on your progress and any changes in your symptoms. If you find that your current treatment is not yielding the desired results, discussing alternative formulas or additional therapies, such as acupuncture, may provide further benefits.
Wishing you the best in your journey towards improved health and well-being. Please feel free to reach out if you have more questions or need further assistance.
Best regards.
