Urinary issues
A few days ago, I noticed that after urination, there was still urine remaining in the urethra of the penis, and I couldn't completely empty it.
Over the next few days, I occasionally found that a drop of urine would leak out while working, eating, or even passing gas.
The frequency of urination has increased; I feel the urge to urinate, and while I can urinate, it feels weaker and I can't fully control it.
Additionally, I have completely lost my libido.
Today, I started doing Kegel exercises and feel somewhat better, with less frequent urination.
However, I have started to experience a slight burning sensation in the urethra, from just below the testicles to the penis, although it is not severe.
My urine appears slightly more yellow, the volume is less, and I am urinating less frequently.
The leakage has improved somewhat, but it is still present.
What could be the cause of this?

Dr. Xu Weikai reply Urology
Hello, this situation may be caused by infection, benign prostatic hyperplasia, anxiety, or cold weather.
It is recommended to treat it as an infection initially: increase your fluid intake, and if possible, check your urine to rule out many possibilities.
Additionally, yellow urine indicates decreased hydration, so it is important to increase your fluid intake to improve this condition.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

More Info
Understanding urinary issues in men can be complex, as they may stem from various underlying causes, including infections, anatomical changes, or even psychological factors. Based on the symptoms you've described, it seems you are experiencing a combination of urinary retention, incontinence, and possibly some degree of discomfort or irritation in the urinary tract.
Possible Causes
1. Urinary Tract Infection (UTI): One of the most common causes of urinary discomfort and changes in urination patterns is a UTI. Symptoms can include a burning sensation during urination, increased frequency of urination, and sometimes even leakage. The fact that you mentioned a slight burning sensation and changes in urine color could suggest a possible infection.
2. Prostate Issues: Given your age and the symptoms of incomplete bladder emptying and increased frequency, benign prostatic hyperplasia (BPH) could be a contributing factor. BPH is common in older men and can lead to urinary retention, urgency, and incontinence.
3. Neurological Factors: Conditions affecting the nervous system can impact bladder control. If there is any nerve damage or dysfunction, it could lead to symptoms like difficulty starting urination or feeling like the bladder is not fully emptying.
4. Psychological Factors: Stress and anxiety can also manifest as urinary issues. If you find yourself in a high-stress environment, it could exacerbate your symptoms.
5. Lifestyle Factors: Dehydration can lead to concentrated urine, which may appear darker and can cause irritation. Additionally, certain foods and drinks, such as caffeine and alcohol, can irritate the bladder and worsen symptoms.
Recommended Solutions
1. Increase Hydration: Ensure you are drinking enough water throughout the day. This can help dilute your urine and may alleviate some of the burning sensations and discomfort.
2. Pelvic Floor Exercises: You mentioned starting Kegel exercises, which can be beneficial. These exercises strengthen the pelvic floor muscles, which can help improve bladder control and reduce leakage.
3. Medical Evaluation: It is crucial to consult a healthcare provider, preferably a urologist, for a thorough evaluation. They may recommend tests such as urinalysis, ultrasound, or urodynamic studies to assess bladder function and rule out any serious conditions.
4. Medication: Depending on the diagnosis, medications may be prescribed to help manage symptoms. For example, alpha-blockers can help relax the muscles around the prostate and bladder neck, making urination easier.
5. Avoid Irritants: Try to identify and avoid any foods or drinks that may irritate your bladder. Common irritants include caffeine, alcohol, and spicy foods.
6. Regular Check-ups: Regular follow-ups with your healthcare provider can help monitor your condition and adjust treatment as necessary.
Conclusion
Your symptoms indicate that there could be multiple factors at play, and it’s essential to approach this issue holistically. While lifestyle changes and pelvic floor exercises can provide some relief, a thorough medical evaluation is crucial to identify any underlying conditions that may require specific treatments. Don’t hesitate to seek medical advice, as early intervention can significantly improve your quality of life and prevent further complications.
