Allergic Reactions to Skin Treatments in Plastic Surgery - Plastic Surgery

Allergy

On Monday, I went to the hospital to get medication for multiple abrasions on my face.
The original wounds were treated with Bacitracin Neomycin ointment, and some of the wounds had healed, so I applied two whitening creams: one was Elomet and the other was called Melquine.
I applied them on Monday night.
On Tuesday morning, there were no issues, but when I woke up on Wednesday, the areas where I applied the ointment were red.
I consulted a dermatologist, who said it was an allergic reaction to the medication.
They prescribed me some oral medication.
I have now stopped using the two whitening creams, but the areas on my face where I applied the medication are still red.
Should I apply Bacitracin Neomycin ointment to the red areas? Will the redness on my face improve more quickly?

Dr. Zhou Hongzhang reply Plastic Surgery

1.
Approximately 5% of individuals of East Asian descent may experience an allergic reaction to "whitening" creams.
If an allergic reaction occurs, it may be necessary to discontinue use or reduce the dosage.

2.
It is normal for a reddish appearance to occur after a facial abrasion, as this is part of the natural scar maturation process, which typically takes about 2 weeks to 2 months to fade.
However, there may be an initial phase of hyperpigmentation, so it is essential to maintain strict sun protection and skincare; otherwise, there is a risk of significant pigmentation issues in the future.

Understanding allergic reactions to skin treatments, especially in the context of plastic surgery, is crucial for ensuring optimal healing and minimizing complications. In your case, it appears that you have experienced an allergic reaction to the topical medications you applied to your face, specifically the whitening creams Elomet and Melquine.
Elomet contains mometasone furoate, a potent topical corticosteroid, which can sometimes cause skin irritation or allergic reactions, particularly if used on sensitive areas or for prolonged periods. Melquine, on the other hand, may contain ingredients that can provoke allergic responses in some individuals. The redness you observed after applying these creams suggests that your skin may be reacting negatively to one or both of these products.

When dealing with allergic reactions, the first step is to discontinue the use of any suspected irritants or allergens, which you have already done. It's also important to follow your dermatologist's advice regarding any prescribed medications to manage the allergic reaction. These may include antihistamines to alleviate itching and inflammation, or topical corticosteroids to reduce redness and swelling.

Regarding your question about using Bacitracin Neomycin (利膚軟膏) on the red areas, it’s essential to approach this with caution. Bacitracin Neomycin is an antibiotic ointment that can help prevent infection in open wounds, but it may not be suitable for areas already exhibiting signs of an allergic reaction. Applying it to inflamed skin could potentially worsen the irritation or lead to further allergic responses, especially since Neomycin is known to cause contact dermatitis in some individuals.

Instead, consider the following steps for managing the red areas on your face:
1. Avoid Further Irritation: Do not apply any products that could further irritate the skin. This includes avoiding harsh cleansers, exfoliants, or any new topical treatments until the redness subsides.

2. Moisturize Gently: Use a gentle, hypoallergenic moisturizer to keep the skin hydrated. Look for products that are fragrance-free and formulated for sensitive skin.

3. Cool Compresses: Applying cool, damp cloths to the affected areas can help soothe the skin and reduce redness.

4. Follow-Up with Your Dermatologist: If the redness persists or worsens, it’s crucial to return to your dermatologist for further evaluation. They may recommend alternative treatments or additional medications to help with the healing process.

5. Monitor for Infection: Keep an eye on the affected areas for any signs of infection, such as increased redness, swelling, warmth, or pus. If these symptoms occur, seek medical attention promptly.

In summary, while Bacitracin Neomycin can be beneficial for preventing infection, it may not be the best choice for treating areas of allergic reaction. Focus on gentle care, moisturizing, and consulting with your healthcare provider for the best course of action. With proper care and attention, the redness should improve over time. Remember, skin healing can be a gradual process, and patience is key.
