Abdominal discomfort or pain?
Hello, Dr.
Lai.
Recently, I have been experiencing occasional dull pain in the upper right abdomen (approximately below the chest).
After having a cold in April, I developed a severe cough that took several weeks to fully recover from.
However, after recovering from the cough, I started to experience this occasional dull pain in the upper right abdomen.
Could this pain be related to the previous cough? (Note: I have a history of allergic rhinitis; could this be a contributing factor?) My allergy symptoms have also been worsening lately, and I often feel a heaviness on the top of my tongue (as if there is a lot of mucus causing nasal congestion, which leads to coughing again).
I also frequently sneeze.
Are these symptoms consistent with allergic rhinitis? (I have never experienced this heaviness on the top of my tongue before.) Thank you very much for your response.

Hello: In response to your question, persistent coughing may lead to inflammation of the intercostal muscles or the diaphragm, resulting in pain.
Additionally, inflammation of the lower lung lobes may also cause pain; it is recommended to seek evaluation from a physician.
Furthermore, post-nasal drip can irritate the throat and trigger coughing symptoms, which is a common manifestation of allergic rhinitis.
It may also be indicative of chronic rhinitis or sinusitis, so please consult an otolaryngologist for diagnosis and treatment.

More Info
Upper right abdominal discomfort following a period of severe coughing can be attributed to several potential causes, particularly in the context of your recent respiratory illness and underlying allergic rhinitis.
Firstly, persistent coughing can lead to muscle strain, particularly in the intercostal muscles (the muscles between the ribs) and the diaphragm. This strain can result in localized pain in the upper right abdomen, especially if the coughing was forceful or prolonged. The diaphragm, which separates the chest cavity from the abdominal cavity, can become irritated or inflamed due to excessive coughing, leading to discomfort that may be felt in the upper abdominal region.
Additionally, your history of allergic rhinitis could contribute to your symptoms. Allergic rhinitis often leads to post-nasal drip, where mucus accumulates in the throat and can trigger coughing. This chronic irritation can exacerbate coughing and may also lead to throat discomfort, which could be perceived as a heavy sensation in the upper part of the tongue or throat. The increased mucus production and nasal congestion can also lead to a feeling of pressure or discomfort in the upper respiratory tract, which may indirectly affect the diaphragm and surrounding muscles.
Moreover, if you have experienced any recent respiratory infections, such as bronchitis or pneumonia, these conditions can also cause referred pain to the upper abdomen. Inflammation of the lungs or pleura (the lining around the lungs) can lead to pain that radiates to the abdomen, particularly when coughing or taking deep breaths.
Given your symptoms, it is essential to consider other potential causes of upper right abdominal pain, such as gallbladder issues (like cholecystitis), liver problems, or even kidney stones. However, these conditions typically present with additional symptoms, such as fever, jaundice, or changes in urine output, which you have not mentioned.
In terms of management, it is advisable to consult a healthcare provider, particularly a gastroenterologist or an ear, nose, and throat (ENT) specialist, to evaluate your symptoms further. They may recommend imaging studies, such as an ultrasound or CT scan, to rule out any underlying structural issues in the abdomen. Additionally, they may assess your allergic rhinitis and consider treatment options to alleviate your nasal symptoms, which could help reduce the frequency and severity of your cough.
In summary, your upper right abdominal discomfort is likely related to muscle strain from coughing, possibly exacerbated by your allergic rhinitis. However, it is crucial to seek medical advice to rule out other potential causes and to receive appropriate treatment for both your abdominal pain and your allergic symptoms.
