After catching a cold, I experience pain in the upper right abdomen when sneezing, running, or laughing?
Hello, I have been coughing for nearly a month and although I have seen a doctor, I am not completely better.
About two weeks ago, I started experiencing noticeable pain in the upper right abdomen near the right side of my waist whenever I cough, run, laugh, or exert myself in that area.
I also feel a dull ache when sitting still.
Could you please advise what this might be and why this is happening? Which specialty should I consult? Thank you!

Dr. Cai Anshun reply Gastroenterology and Hepatology
Hello: You described experiencing a dull pain in the upper right abdomen near the right flank for the past two weeks.
If this pain is accompanied by a persistent cough, especially when taking deep breaths, it may be necessary to rule out pneumonia in the right lower lobe.
Additionally, please consider whether there has been any recent trauma.
Other possibilities include a liver abscess or kidney infection.
However, pneumonia, liver abscess, and kidney infection may all present with recurrent fever.
If the symptoms persist, it is advisable to consult a gastroenterologist for an initial evaluation.

More Info
Right upper abdominal pain after coughing can be a concerning symptom, especially if it persists for an extended period. Given your description of the pain occurring during activities that involve the use of abdominal muscles, such as coughing, laughing, or running, it is essential to consider several potential causes and the appropriate medical specialists to consult.
Possible Causes of Right Upper Abdominal Pain
1. Muscle Strain: One of the most common causes of pain in the abdominal area after prolonged coughing is muscle strain. The intercostal muscles, which are located between the ribs, can become strained due to repetitive coughing. This strain can lead to localized pain that may worsen with movement or certain activities.
2. Diaphragmatic Irritation: Coughing can also irritate the diaphragm, which is the muscle that separates the chest from the abdomen. If the diaphragm is inflamed or irritated, it can cause pain that radiates to the upper abdomen, especially on the right side.
3. Referred Pain from the Lungs: Conditions affecting the lungs, such as pneumonia or pleuritis (inflammation of the lining of the lungs), can cause referred pain to the abdomen. This is due to the shared nerve pathways between the thoracic and abdominal regions.
4. Gallbladder Issues: Pain in the right upper quadrant can also be related to gallbladder problems, such as gallstones or cholecystitis (inflammation of the gallbladder). This pain may be exacerbated by movements that engage the abdominal muscles.
5. Hernia: If there is a history of abdominal surgery or a known hernia, this could also be a factor. A hernia can become painful with increased abdominal pressure, such as during coughing.
6. Gastrointestinal Issues: Conditions such as gastritis, peptic ulcers, or even liver issues can manifest as pain in the right upper abdomen.
Recommended Specialists
Given the range of potential causes, it is advisable to consult with a healthcare professional who can perform a thorough evaluation. Here are some specialists you might consider:
- Primary Care Physician: Start with your primary care doctor, who can assess your symptoms, perform a physical examination, and order initial tests such as blood work or imaging studies (like an ultrasound or CT scan) if necessary.
- Gastroenterologist: If gastrointestinal issues are suspected, a gastroenterologist can provide specialized care for conditions affecting the digestive system.
- Pulmonologist: If there is a concern about lung-related issues, a pulmonologist can evaluate respiratory conditions that might be contributing to your symptoms.
- Orthopedic or Pain Specialist: If muscle strain or musculoskeletal issues are suspected, consulting with an orthopedic specialist or a pain management physician may be beneficial.
Conclusion
In summary, right upper abdominal pain after coughing can arise from various causes, including muscle strain, diaphragmatic irritation, referred pain from lung conditions, gallbladder issues, hernias, or gastrointestinal problems. It is crucial to seek medical attention to determine the underlying cause of your symptoms. A thorough evaluation by a primary care physician is a good starting point, and they can guide you to the appropriate specialist based on their findings. Early diagnosis and treatment can help alleviate your pain and address any underlying health issues effectively.
