Loud sounds in the ears when swallowing saliva?
Hello, doctor: I have a question.
When I swallow saliva, there is a loud sound in my ears that resembles bones breaking.
It gets worse when I go uphill, and sometimes I hear a popping sound.
At times, it feels like something is pushing in and causing a painful thud.
Swallowing doesn't help; in fact, it makes my ears hurt more.
What could be the problem? I'm hesitant to swallow saliva, especially when going uphill.
How is this related to the temporomandibular joint (TMJ)? When will the noise decrease? Thank you.

Dear Inquirer: Hello, based on your description, it may be caused by Eustachian tube dysfunction.
What exactly is the cause of this dysfunction? I recommend visiting a nearby clinic for diagnosis and treatment.
Sincerely, Dr.

More Info
The symptoms you are experiencing, particularly the loud sounds in your ears when swallowing, as well as the sensations of pressure and pain, can be quite distressing. These symptoms may be indicative of several underlying issues, primarily related to the Eustachian tube function, middle ear pressure, and possibly the temporomandibular joint (TMJ) dysfunction.
Understanding the Causes
1. Eustachian Tube Dysfunction: The Eustachian tube connects the middle ear to the back of the throat and helps equalize ear pressure. When you swallow, the muscles around the Eustachian tube contract, allowing air to flow into the middle ear and equalizing pressure. If the Eustachian tube is not functioning properly, it can lead to a sensation of fullness, pressure, or pain in the ear, especially during changes in altitude, such as when climbing a mountain. The loud sounds you hear could be due to the sudden changes in pressure or the movement of fluid or mucus in the ear.
2. Middle Ear Issues: If there is fluid buildup in the middle ear, it can create a sensation of fullness and lead to abnormal sounds when swallowing. This could be due to allergies, infections, or other conditions that affect the Eustachian tube's ability to ventilate the middle ear properly.
3. TMJ Dysfunction: The temporomandibular joint is located near the ear, and issues with this joint can cause referred pain or sounds in the ear. If you are experiencing jaw pain or clicking, it may be related to TMJ dysfunction, which can also affect your swallowing and create unusual sounds in the ear.
Solutions and Recommendations
1. Consult an Ear, Nose, and Throat (ENT) Specialist: Given the complexity of your symptoms, it is advisable to consult an ENT specialist. They can perform a thorough examination, including a tympanometry test to assess middle ear pressure and Eustachian tube function.
2. Eustachian Tube Exercises: You can try techniques to help open the Eustachian tube, such as the Valsalva maneuver (pinching your nose and gently blowing) or the Toynbee maneuver (pinching your nose and swallowing). However, if these maneuvers cause pain, it is best to avoid them until you have consulted a doctor.
3. Manage Allergies and Sinus Issues: If allergies or sinus congestion are contributing to your symptoms, consider using antihistamines or nasal decongestants as recommended by your healthcare provider.
4. TMJ Management: If TMJ dysfunction is suspected, treatments may include physical therapy, dental splints, or stress management techniques to reduce jaw tension.
5. Avoid Sudden Altitude Changes: Until you have a clearer understanding of your condition, try to avoid rapid changes in altitude, as this can exacerbate your symptoms.
6. Pain Management: Over-the-counter pain relievers may help alleviate discomfort, but consult your doctor before starting any new medication.
Conclusion
The sounds you hear in your ears when swallowing, along with the associated pain and pressure, are likely related to Eustachian tube dysfunction or possibly TMJ issues. It is essential to seek medical advice to determine the exact cause and appropriate treatment. With proper evaluation and management, you can find relief from these symptoms and improve your quality of life.
