Nasal cry (or rhinorrhea)?

Hello Doctor: I have been struggling with post-nasal drip for over two years, and it has been persistent without any relief.
I have seen many doctors and have tried antibiotics, antihistamines, and nasal sprays, but none have been effective.
I have undergone nasal endoscopy and X-rays, but no issues were found, and I was ultimately diagnosed with allergies.
I feel that there is a constant flow of fluid from the upper part of my right nasal cavity, near the area below my right eye.
The fluid is colorless and transparent but very thick and slippery, which doesn't feel like regular nasal mucus.
Due to its viscosity and constant post-nasal drip, I often experience severe coughing that makes it difficult to speak and can lead to vomiting.
Sometimes, I also experience dizziness and headaches.
This situation has persisted for over two years without improvement.
I would like to ask the doctor if my condition is truly due to allergies.
Is this type of post-nasal drip normal for allergies? What should I do now?

Dear Inquirer,
Based on your literal question, you may not only have chronic rhinitis and sinusitis (allergic), but you might also be experiencing chronic pharyngitis.
Would you consider visiting my clinic for assistance? My clinic hours at Taoyuan Hospital, Department of Health, are Tuesday mornings, Wednesday afternoons, and Thursday mornings.
Sincerely,
Dr.

More Info

Nasal drip, or post-nasal drip, is a common condition that can significantly affect a person's quality of life. It occurs when excess mucus accumulates in the back of the throat, often leading to a sensation of mucus dripping down from the nasal passages. This condition can be particularly bothersome, as it may cause coughing, throat irritation, and even nausea in severe cases. Understanding the causes, symptoms, and treatment options for nasal drip is essential for effective management.

Causes of Nasal Drip
1. Allergies: Allergic rhinitis is a prevalent cause of nasal drip. Allergens such as pollen, dust mites, pet dander, and mold can trigger an inflammatory response in the nasal passages, leading to increased mucus production.

2. Infections: Viral infections, such as the common cold or sinusitis, can lead to nasal drip. In these cases, the mucus may become thicker and discolored, indicating a possible bacterial infection.

3. Structural Issues: Anatomical abnormalities, such as a deviated septum or nasal polyps, can obstruct normal mucus drainage, causing accumulation and subsequent dripping.

4. Environmental Factors: Exposure to irritants like smoke, strong odors, or dry air can exacerbate mucus production and contribute to nasal drip.

5. Gastroesophageal Reflux Disease (GERD): Acid reflux can irritate the throat and lead to a sensation of mucus accumulation, mimicking post-nasal drip.

Symptoms of Nasal Drip
The symptoms associated with nasal drip can vary but commonly include:
- A persistent feeling of mucus in the throat
- Frequent throat clearing
- Coughing, especially at night or upon waking
- Sore throat
- Bad breath
- Nausea or vomiting in severe cases
In your situation, the clear, thick, and slippery mucus you describe, along with the persistent cough and associated symptoms like headaches and dizziness, suggests that there may be an underlying issue that requires further investigation.

Treatment Options
1. Medications:
- Antihistamines: These can help reduce allergic reactions and decrease mucus production. Non-drowsy options like loratadine or cetirizine may be beneficial.

- Nasal Corticosteroids: These sprays can reduce inflammation in the nasal passages, helping to alleviate symptoms. Examples include fluticasone or mometasone.

- Decongestants: These can provide temporary relief by reducing swelling in the nasal passages, but they should be used cautiously and not for extended periods.

- Saline Nasal Irrigation: Rinsing the nasal passages with saline can help clear out mucus and allergens, providing symptomatic relief.

2. Lifestyle Modifications:
- Avoiding Triggers: Identifying and avoiding allergens or irritants that exacerbate your symptoms can be crucial.

- Staying Hydrated: Drinking plenty of fluids can help thin mucus, making it easier to drain.

- Humidifiers: Using a humidifier can add moisture to the air, which may help soothe irritated nasal passages.

3. Further Evaluation:
- Given that you have not found relief with standard treatments and have undergone nasal endoscopy and X-rays without definitive findings, it may be worth consulting an allergist or an ENT specialist for further evaluation. They may recommend allergy testing or imaging studies to identify any underlying issues that could be contributing to your symptoms.

4. Consideration of GERD: If you suspect that acid reflux may be contributing to your symptoms, discussing this with your healthcare provider could lead to appropriate management strategies.

Conclusion
In summary, nasal drip can be a frustrating condition, particularly when it persists despite treatment. Your symptoms suggest that there may be an underlying allergic or structural component contributing to your condition. It is essential to work closely with your healthcare provider to explore all potential causes and develop a comprehensive treatment plan tailored to your specific needs. If standard treatments have not been effective, seeking a second opinion or further specialized evaluation may provide new insights and options for relief.
