Questions about ejaculation and urinary incontinence?
Hello doctor, I have been experiencing this issue for a while.
Recently, after becoming intimate with my girlfriend, I noticed that after ejaculation, I tend to have urinary incontinence.
This usually starts about 20 to 30 minutes after resting and can last for 2 to 3 days.
During this time, I find myself leaking urine unexpectedly or experiencing frequent urination (2 to 3 times an hour, but the volume is not much).
Sometimes, even before I reach the restroom, my underwear gets wet.
I never had this issue before I started dating; even after self-stimulation, I would experience urinary incontinence.
Is it possible that my previous excessive indulgence has led to this? Now that I have a girlfriend and the frequency of ejaculation has increased, the urinary incontinence seems to have worsened.
This problem usually resolves itself in two to three days, but it continues to disrupt my life.
If I want to make an appointment, I’m not sure how to bring it up.
Should I just directly see a urologist? After ejaculation, I also feel a pressure in my rectum.
Could this be a sign of something wrong, or is it something that can only be managed with medication? I would appreciate your online response, doctor.

Ejaculation and urinary incontinence are two interconnected issues that can cause significant distress for individuals experiencing them. In your case, it seems that the onset of urinary incontinence following ejaculation has become a source of concern, particularly as it affects your daily life and activities. Let's break down the potential causes and considerations regarding your symptoms.
Firstly, it's important to understand that ejaculation and urination involve the same anatomical structures, specifically the urethra, which serves as the passage for both urine and semen. After ejaculation, the body goes through a refractory period, during which various physiological changes occur. Some individuals may experience a temporary sensation of fullness or pressure in the pelvic region, which could explain the feeling of "pressure" in your anus after ejaculation.
The urinary incontinence you describe—leaking urine shortly after ejaculation—could be attributed to several factors. One possibility is that the pelvic floor muscles, which support the bladder and help control urination, may be weakened or not functioning optimally. This can lead to difficulties in maintaining urinary control, especially after the physical exertion associated with ejaculation. Engaging in pelvic floor exercises, such as Kegel exercises, can help strengthen these muscles and improve control over urination.
Another consideration is that increased sexual activity and ejaculation frequency may lead to temporary changes in bladder sensitivity or function. Some individuals may find that their bladder becomes more reactive or sensitive, leading to increased urgency or frequency of urination. This could explain why you are experiencing more frequent trips to the bathroom after becoming sexually active.
It's also worth noting that psychological factors can play a role in urinary incontinence. Anxiety or stress related to sexual performance or the fear of incontinence itself can exacerbate symptoms. If you find that your symptoms are causing significant distress or impacting your quality of life, it may be beneficial to speak with a healthcare professional who can provide guidance and support.
Regarding your concerns about potential underlying conditions, it is essential to consult a urologist or a healthcare provider specializing in urinary health. They can conduct a thorough evaluation, including a physical examination and possibly some diagnostic tests, to rule out any underlying issues such as urinary tract infections, prostate problems, or other medical conditions that could contribute to your symptoms.
When you visit a healthcare provider, it is helpful to be open and honest about your symptoms. You can start by explaining that you have been experiencing urinary incontinence following ejaculation and any other related symptoms, such as the sensation of pressure in the pelvic area. This will allow the provider to understand your concerns and tailor their evaluation and recommendations accordingly.
In summary, while urinary incontinence after ejaculation can be concerning, it is often manageable with appropriate interventions. Strengthening pelvic floor muscles, addressing any psychological factors, and seeking professional medical advice are crucial steps in finding relief and improving your overall well-being. Don't hesitate to reach out to a urologist, as they can provide the expertise needed to address your specific situation and help you regain confidence in your urinary control.
