Three Questions About Urinary Issues
Hello Dr.
Liu,
I have three personal questions I would like to ask:
1.
After ejaculation during masturbation, most of the semen is expelled, but a small amount leaks out from the urethra afterward.
Is this normal?
2.
After masturbation (with ejaculation), when I go to the bathroom, I sometimes experience difficulty urinating (no longer in an erect state), and there is occasional mild pain.
What could be the cause of this?
3.
I have difficulty completely emptying my bladder during urination, with some residual urine left in the urethra, and I have to exert a lot of effort to expel it.
I often experience urinary leakage when sitting down, standing up, or sneezing.
I have visited a urologist and experienced improvement after taking medication for a few weeks, but the symptoms returned after stopping the medication.
Note: I did not masturbate during the medication period, and I masturbated once after stopping the medication.
Could this be one of the reasons? Additionally, I stopped the medication because I thought I had improved, and I did not return for a follow-up, although the doctor had advised me to come back for one last visit.
Is it necessary to complete the entire treatment course for it to be effective? The medication prescribed seemed to be for bladder weakness.
I would appreciate your answers to these questions.
Thank you!

Don't be so nervous! Congratulations on growing up and being able to drive a manual transmission.
The phenomenon you are experiencing is a normal temporary contraction of the bladder outlet during erection and ejaculation, which may cause a slight feeling of discomfort.
Additionally, it is not possible to completely empty the bladder; having about 50cc of residual urine after urination is generally acceptable.
Moreover, our kidneys continuously produce urine 24 hours a day.
Medication is not necessarily required.
I recommend a book titled "About Sex" (published by Tianxia in Chinese) and "The Love of the Little Brother."

Hello! Thank you for your questions regarding common urological concerns. Let’s address each of your inquiries in detail.
1. Post-Ejaculation Leakage: It is quite common for a small amount of semen to leak from the urethra after ejaculation. This can happen due to residual semen that remains in the urethra after the main ejaculation. The urethra is a tube that carries urine from the bladder and semen from the reproductive system, and it can retain a small amount of fluid. As long as this leakage is minimal and not accompanied by pain or discomfort, it is generally considered normal.
2. Discomfort During Urination After Ejaculation: Experiencing discomfort or a burning sensation during urination after ejaculation can be attributed to several factors. One possibility is that the urethra may be slightly irritated from the ejaculation process, which can lead to temporary discomfort. Additionally, if you are experiencing any inflammation or infection in the urinary tract, this could also contribute to pain during urination. If this discomfort persists or worsens, it would be advisable to consult a healthcare professional for further evaluation.
3. Incomplete Urination and Urinary Incontinence: The sensation of incomplete urination, along with urinary leakage when sitting, standing, or sneezing, can be indicative of a few underlying issues. One common cause is bladder dysfunction, which can lead to difficulty fully emptying the bladder. This may be related to muscle control issues or an obstruction in the urinary tract. The fact that you experienced improvement while on medication suggests that the treatment was addressing an underlying condition, possibly related to bladder muscle strength or coordination.
It is crucial to complete the entire course of prescribed medication, as stopping prematurely can lead to a recurrence of symptoms. Medications for bladder issues often require consistent use to effectively strengthen the bladder muscles and improve function. If you believe that your symptoms have returned after stopping the medication, it would be wise to follow up with your urologist. They may recommend further evaluation or a different treatment plan to address your ongoing concerns.
In summary, while some of the symptoms you are experiencing can be common and may not indicate a serious issue, it is essential to monitor them closely. If you notice any changes, such as increased pain, changes in urinary habits, or any other concerning symptoms, please seek medical attention. Urological health is important, and addressing these concerns with a healthcare professional can lead to effective management and improvement in your quality of life. Thank you for your questions, and I hope this information helps clarify your concerns!
