Testicular pain
Hello, doctor.
I often experience a situation where if I do not ejaculate for an extended period (almost a month) or if I do not relieve sexual arousal for a few days, I will have swelling and pain in my right testicle.
However, whenever I ejaculate during such times, I feel much more comfortable.
Could you please explain why this happens? Thank you.
A-Ming, 20~29 year old female. Ask Date: 2009/11/11
Dr. Xu Weikai reply Urology
A-Ming: Hello, there are many causes of testicular pain.
Based on your description, it could be due to excessive semen accumulation causing discomfort.
Generally, it can be ignored, and after a period of time, the body will gradually adapt and adjust.

Testicular pain, especially after prolonged periods of sexual arousal without ejaculation, is a common issue that many men experience. This phenomenon can be attributed to several physiological and anatomical factors related to the male reproductive system.
When a man experiences sexual arousal, the body undergoes a series of physiological changes. Blood flow to the genital area increases, leading to an erection and heightened sensitivity in the testicles. This process is part of the body's natural response to sexual stimulation. If ejaculation does not occur, the increased blood flow can lead to a condition known as "blue balls" or epididymal hypertension. This condition is characterized by a feeling of heaviness or aching in the testicles, particularly on the side that is more stimulated, which in your case is the right testicle.
The discomfort arises because the body is essentially in a state of heightened arousal without the release that typically follows ejaculation. The pressure builds up in the testicular area, leading to pain or discomfort. When ejaculation occurs, the body releases built-up tension, and the blood flow returns to normal, alleviating the pain and discomfort. This is why you feel relief after ejaculation.
In some cases, prolonged sexual arousal without ejaculation can also lead to inflammation of the epididymis, the tube that stores and carries sperm. This condition, known as epididymitis, can cause pain and swelling in the testicles. However, if the pain is solely linked to the buildup of pressure and resolves after ejaculation, it is less likely to be a sign of an underlying infection or serious condition.
It is important to note that while occasional discomfort due to prolonged arousal is generally not a cause for concern, persistent or severe testicular pain should be evaluated by a healthcare professional. Other potential causes of testicular pain can include infections, trauma, torsion (twisting of the testicle), or hernias, which may require medical intervention.
If you find that this discomfort occurs frequently or is accompanied by other symptoms such as swelling, redness, or fever, it would be wise to consult a urologist. They can perform a thorough examination and possibly recommend imaging studies to rule out any serious conditions.
In summary, the pain you experience in your right testicle after prolonged sexual arousal without ejaculation is likely due to increased blood flow and pressure in the area, which is relieved upon ejaculation. While this is a common occurrence, if the pain persists or worsens, seeking medical advice is recommended to ensure there are no underlying issues.
