White Discharge from the Glans During Bowel Movements - Urology

Bowel movement

Hello, doctor.
Recently, I have noticed that during bowel movements, my glans secretes a milky white fluid.
At that moment, I do not experience any sensation of ejaculation, but after performing the ejaculation motion, the glans stops secreting the fluid.
I have not engaged in any sexual activity for the past six months, and there are no abnormalities with my glans.
My urination also appears normal.
Could you please tell me what that fluid is? Is it prostatic fluid? Do I need to see a doctor? Thank you for your assistance.

Tan, 30~39 year old female. Ask Date: 2009/11/19

Dr. Xu Weikai reply Urology

Hello, the presence of discharge from the urethra can be attributed to two possibilities: 1.
Normal glandular secretion: There are many glands near the urethra that normally produce secretions.
2.
Abnormal conditions: Infections and other issues need to be differentiated, and it is not possible to identify them based solely on appearance.
It is best to conduct further examinations, such as analysis and culture of the discharge.
If you have any further questions or need clarification, feel free to ask, and we can discuss and research together.
Sincerely, Dr.
Hsu Wei-Kai, Director of Urology, Taitung Hospital, Department of Health, Executive Yuan.

The presence of a white discharge from the glans during bowel movements can be concerning, and it's important to understand what this could indicate. Based on your description, it seems that the discharge occurs when you are straining during bowel movements, and it stops after you simulate an ejaculation. This could suggest that the fluid is related to the prostate or seminal vesicles, which can produce fluid that may be expelled under certain conditions, such as increased abdominal pressure during bowel movements.

The fluid you are experiencing could potentially be prostatic fluid. The prostate gland produces a significant portion of the fluid that makes up semen, and it can sometimes release this fluid in response to physical pressure or stimulation, even if there is no sexual arousal or ejaculation. This is not uncommon, especially if you are experiencing any straining during bowel movements.

It is also worth noting that if the discharge is clear or milky and does not have a foul odor, it is less likely to be a sign of an infection. However, if the discharge has a strong odor, is accompanied by pain, itching, or any other unusual symptoms, it may indicate an infection or other medical condition that requires attention.

In your case, since you mentioned that you have not engaged in sexual activity for the past six months and there are no other symptoms such as pain during urination or changes in urinary habits, it may not be an immediate cause for concern. However, it is always advisable to consult a healthcare professional if you notice any changes in your body that are unusual for you. A physician can perform a thorough examination and may recommend tests to rule out any underlying conditions, such as prostatitis or other urogenital issues.

In summary, while the white discharge you are experiencing could be related to prostatic fluid, it is essential to monitor your symptoms. If you notice any changes, such as an increase in discharge, changes in color or odor, or any discomfort, it would be prudent to seek medical advice. Regular check-ups and open communication with your healthcare provider can help ensure that any potential issues are addressed promptly and effectively.
