Laceration of the foreskin?


Hello Doctor, for the past year, I have experienced slight fissures at the tip of my foreskin whenever I masturbate or have intercourse with my girlfriend.
The wounds generally heal in about three days, but they are very painful when exposed to water during that time.
I never had this issue before, but now it happens every time.
Upon observation, I noticed that the elasticity at the tip of my foreskin seems to have significantly decreased, making it difficult for the foreskin to retract.
Even when I am careful to retract it, I still experience fissures during intercourse.
I am wondering if there is any ointment I should apply to improve this condition.
