Concerns About Testicular Health and Fertility After DIY Activities - Urology

I have concerns?

Hello Dr.
Liu, I have a question.
When I was 18 years old, after a DIY session, I immediately went to urinate, and my testicles suddenly swelled.
Since then, I feel like my sperm has lost its viscosity.
Will this lead to infertility in the future? After that incident, I also find it difficult to achieve an erection naturally.
Could this be related? I notice that after DIY or when I come home tired from going out, my penis feels very relaxed, but on days when I don’t DIY, it feels tight.
Additionally, when I exert myself at work, my testicles feel uncomfortable, and after urinating, my penis seems to droop.
What kind of medication should I take?

You're overthinking it! It's normal! No need for medication.
If you're worried, you can visit a urologist at the hospital.

Concerns regarding testicular health and fertility are valid and can be influenced by various factors, including physical activities, stress, and underlying medical conditions. Based on your description, it seems you experienced a significant event at the age of 18 that has led to ongoing concerns about your reproductive health. Let's break down your concerns and provide some insights.

Firstly, the incident you described—experiencing sudden swelling of the testicles after a DIY activity—could have been indicative of a few potential issues, such as trauma, inflammation, or even a torsion (twisting) of the testicle, which is a medical emergency. If this swelling was significant and accompanied by pain, it would have warranted immediate medical attention. However, if the swelling subsided and you did not experience any further acute symptoms, it may not have led to long-term damage.
Regarding your concerns about sperm quality, it is important to note that sperm health can be influenced by various factors, including temperature, lifestyle, and overall health. The perception that your sperm has "no stickiness" could be related to changes in your body or anxiety about your fertility. Sperm motility and morphology can be affected by stress, hormonal imbalances, and other health conditions. If you are concerned about fertility, a semen analysis conducted by a healthcare professional can provide valuable insights into your sperm health.

As for the changes in your erections and the feeling of relaxation in your genital area, these could be linked to psychological factors such as anxiety or stress, especially if you associate these feelings with the incident from your past. Psychological factors can significantly impact erectile function, leading to difficulties in achieving or maintaining an erection. It is essential to address any underlying anxiety or stress, as these can create a cycle of worry that exacerbates the issue.

The discomfort in your testicles during physical exertion and the sensation of your penis feeling "looser" after certain activities could also be related to muscle tension or pelvic floor issues. The pelvic floor muscles play a crucial role in supporting the pelvic organs, including the testicles and penis. If these muscles are tense or not functioning optimally, it can lead to discomfort or changes in sensation.

In terms of treatment, it is crucial to consult with a healthcare professional, such as a urologist, who can evaluate your symptoms comprehensively. They may recommend lifestyle changes, pelvic floor exercises, or even counseling to address any psychological factors. If there is an underlying medical condition, appropriate treatment can be initiated.

In summary, while your concerns about testicular health and fertility are understandable, it is essential to seek professional medical advice to address these issues accurately. A thorough evaluation can help determine if there are any underlying conditions that need to be treated and provide you with peace of mind regarding your reproductive health. Remember, open communication with your healthcare provider is key to addressing your concerns effectively.
