Bowel issues
Hello, doctor...
I have never been able to have a daily bowel movement.
When I do go, it is often diarrhea, and I frequently feel bloated to the point of discomfort.
I usually take over-the-counter probiotics and green tea in the morning to help with bowel movements, as I often experience gas.
Recently, I stopped taking probiotics and green tea, which resulted in very hard stools.
In an effort to relieve myself, I experienced anal dilation that led to bleeding.
Interestingly, the first part of my stool is very hard, while the latter part is more like diarrhea.
I often eat out, and despite consuming a lot of fruits, it doesn't seem to help.
Additionally, I spend long hours sitting in an office.
I would like to know if there are any methods to improve this condition.

Dr. Lin Minghui reply Gastroenterology and Hepatology
Hello, if you have not had a bowel movement for several days, are experiencing difficulty with bowel movements, frequent bloating, and loose stools, I recommend the following:
1.
Develop good dietary habits - chew your food thoroughly, avoid eating too quickly, eat until you are about 80% full, and have regular meal times.
If you experience bloating, limit your intake of carbonated beverages, gas-producing foods, and spicy foods.
2.
Establish a regular bowel habit - try to sit on the toilet at the same time every day, as many people find it easier to have a bowel movement before breakfast.
Ensure you are adequately hydrated and consume enough fiber.
Drinking yogurt can also help replenish probiotics.
If your stools are too hard, it may lead to anal fissures or bleeding from hemorrhoids.
3.
Engage in moderate exercise - even if you work in an office, it is advisable not to sit for prolonged periods, as this can contribute to hemorrhoids.
Consider doing some stretching exercises or engaging in physical activity on weekends.
Maintain a positive mood and a regular daily routine.
Wishing you good health,
Lin Ming-Hui, Department of Health, Taoyuan Hospital

Chronic bowel issues, such as constipation and diarrhea, can be quite distressing and are often linked to dietary habits, lifestyle choices, and underlying health conditions. Based on your description, it seems you are experiencing a combination of symptoms that may indicate an imbalance in your digestive system. Here are some effective strategies and tips to help alleviate your chronic bowel issues.
1. Dietary Adjustments
- Increase Fiber Intake: Aim for a diet rich in both soluble and insoluble fiber. Soluble fiber, found in foods like oats, beans, and fruits, helps to soften stool, while insoluble fiber, found in whole grains and vegetables, adds bulk to stool. The recommended daily intake of fiber is about 25-30 grams.
- Hydration: Drink plenty of water throughout the day. Aim for at least 1.5 to 2 liters (about 6-8 cups) daily. Proper hydration helps to soften stool and promotes regular bowel movements.
- Limit Processed Foods: Processed foods often lack fiber and can contribute to digestive issues. Try to prepare meals at home using whole, unprocessed ingredients whenever possible.
- Probiotics: Incorporating probiotics, such as yogurt or fermented foods, can help restore the natural balance of gut bacteria, which is essential for healthy digestion. If you find that over-the-counter probiotics are beneficial, consider continuing their use.
2. Regular Meal Patterns
- Consistent Eating Schedule: Try to eat meals at the same time each day. This can help regulate your digestive system and promote regular bowel movements.
- Avoid Skipping Meals: Skipping meals can lead to irregular bowel habits. Ensure you have three balanced meals a day, and consider healthy snacks if needed.
3. Physical Activity
- Exercise Regularly: Physical activity stimulates digestion and can help alleviate constipation. Aim for at least 30 minutes of moderate exercise most days of the week. Activities like walking, jogging, or yoga can be particularly beneficial.
- Take Breaks from Sitting: If you have a sedentary job, make it a point to stand up, stretch, or take short walks every hour to promote circulation and digestive health.
4. Mindful Eating
- Chew Thoroughly: Take your time to chew your food well. This aids in digestion and can help prevent bloating and discomfort.
- Avoid Overeating: Eating large meals can lead to discomfort and bloating. Try smaller, more frequent meals instead.
5. Managing Stress
- Stress Reduction Techniques: Stress can significantly impact digestive health. Consider incorporating relaxation techniques such as deep breathing, meditation, or yoga into your daily routine.
6. Monitor Symptoms
- Keep a Food Diary: Tracking what you eat and your bowel movements can help identify any food intolerances or patterns that may be contributing to your symptoms.
- Consult a Healthcare Professional: If your symptoms persist despite making these changes, it is crucial to consult with a healthcare provider. They may recommend further testing to rule out conditions such as Irritable Bowel Syndrome (IBS), inflammatory bowel disease, or food intolerances.
7. Medications and Supplements
- Over-the-Counter Solutions: If dietary changes are not sufficient, consider discussing the use of over-the-counter laxatives or stool softeners with your healthcare provider. However, these should not be used as a long-term solution without medical supervision.
- Prescription Medications: In some cases, prescription medications may be necessary to manage chronic bowel issues effectively.
In conclusion, addressing chronic bowel issues often requires a multifaceted approach that includes dietary changes, regular physical activity, stress management, and possibly medical intervention. By implementing these strategies, you can work towards achieving more regular bowel movements and improving your overall digestive health. If symptoms persist or worsen, it is essential to seek professional medical advice for further evaluation and tailored treatment options.
