Foreign body in the throat
Hello, doctor.
I would like to ask about a situation where the saliva expelled from my throat has a foul odor.
Sometimes when I have a sore throat, I feel like there are white, rice-like remnants stuck in my throat.
When I expel them, they are yellowish and smell very bad.
What could this be, and could it indicate a pathological condition in my throat? Thanks.
Yu

Dear Yu,
Thank you for your inquiry regarding the unpleasant odor and pain associated with your throat. Based on your description, it sounds like you may be experiencing a condition known as tonsilloliths, or more commonly referred to as tonsil stones. These are calcified deposits that can form in the crevices of the tonsils and can lead to a foul smell, especially when they are expelled or dislodged.
Tonsilloliths are often composed of bacteria, dead cells, and food particles that become trapped in the tonsils. When these materials accumulate, they can harden and form stones, which can cause discomfort, a sensation of something being stuck in the throat, and bad breath. The yellowish color you mentioned is typical of these stones, and the odor is a result of the bacterial activity and decomposition of organic material.
In addition to tonsilloliths, other potential causes for the symptoms you are experiencing could include chronic tonsillitis, which is an inflammation of the tonsils that can lead to recurrent sore throats and the formation of pus. This condition can also produce a foul smell and may be accompanied by white or yellow patches on the tonsils.
Another possibility is post-nasal drip, where mucus from the nasal passages drips down the back of the throat, leading to irritation and a buildup of bacteria that can cause bad breath and discomfort. If you have been experiencing nasal congestion or sinus issues, this could be a contributing factor.
It's also important to consider other conditions such as pharyngitis or laryngitis, which can cause throat pain and may be associated with infections, either viral or bacterial. In some cases, a more serious condition such as a throat abscess or even malignancy could present with similar symptoms, although these are less common.
To address your concerns, I recommend the following steps:
1. Consult an ENT Specialist: Given your symptoms, it would be prudent to see an Ear, Nose, and Throat (ENT) specialist. They can perform a thorough examination of your throat and tonsils, possibly using a scope to get a better view of any underlying issues.
2. Maintain Oral Hygiene: Good oral hygiene can help minimize the formation of tonsil stones and reduce bad breath. Regular brushing of your teeth and tongue, along with gargling with salt water, may help alleviate some symptoms.
3. Stay Hydrated: Drinking plenty of fluids can help thin mucus and reduce the likelihood of post-nasal drip, which may contribute to your symptoms.
4. Consider Tonsillectomy: If you find that tonsilloliths or chronic tonsillitis are a recurring issue, your doctor may discuss the option of a tonsillectomy, which is the surgical removal of the tonsils.
5. Monitor Symptoms: Keep track of any additional symptoms you may experience, such as fever, difficulty swallowing, or persistent pain, and report these to your healthcare provider.
In conclusion, while your symptoms may be indicative of a relatively common condition such as tonsilloliths or chronic tonsillitis, it is essential to seek professional medical advice for an accurate diagnosis and appropriate treatment. Your health and comfort are important, and addressing these symptoms early can lead to better outcomes.
