I'm not sure which specialty to consult or what tests to undergo?
Hello, doctor.
My diet is generally normal; I usually have three meals a day, although I occasionally skip breakfast on weekends.
My weight is within the standard range (160 cm / 49 kg), but I appear "thin" (like under 45 kg) and look like I’m not eating.
However, my weight does fluctuate.
On regular workdays, my diet is fine, but on weekends, I always experience diarrhea within 15 minutes after my first meal after waking up.
I also get easily fatigued; even if I sleep in, if I engage in any activity (even just sitting) for more than about three hours, I feel very tired and my eyes become strained.
Usually, a short nap helps a bit.
My cheeks appear sunken, and I consistently have dark circles under my eyes (I have a habit of drinking coffee on weekdays, but I don’t have any bad habits).
In the past, I would catch a cold about once every 1-2 months, but in the last two years, the frequency has decreased significantly (1-2 times a year).
When I do catch a cold, it tends to be severe, taking about 2-3 weeks to recover.
A friend pointed out that my physical condition seems unusual, but I’m not sure which specialist to see or what tests to undergo.
I would appreciate your guidance.

It seems that the symptoms cannot rule out irritable bowel syndrome or functional gastrointestinal disorders; it would be advisable to consult a gastroenterologist first.

More Info
Hello! Thank you for reaching out with your concerns regarding your health. Based on the symptoms you've described, it seems you are experiencing a range of issues that could be interconnected. Let's break down your symptoms and discuss the potential specialists you might consider consulting.
Symptoms Overview
1. Digestive Issues: You mentioned that you often experience diarrhea shortly after your first meal of the day. This could indicate a gastrointestinal issue, such as irritable bowel syndrome (IBS), food intolerances, or even an infection.
2. Fatigue and Weakness: Feeling fatigued after minimal activity and experiencing persistent tiredness can be indicative of several underlying conditions, including anemia, thyroid dysfunction, or chronic fatigue syndrome.
3. Physical Appearance: The observation that your cheeks appear sunken and the presence of dark circles under your eyes could be related to dehydration, lack of sleep, or nutritional deficiencies.
4. Frequent Illness: While you noted a decrease in the frequency of colds, the severity of your symptoms when you do get sick is concerning. This could suggest an underlying issue with your immune system or overall health.
Recommended Specialists
Given the complexity of your symptoms, it would be beneficial to consult with the following specialists:
1. Gastroenterologist: Since you are experiencing digestive issues, a gastroenterologist can perform tests such as an endoscopy or colonoscopy to rule out conditions like IBS, inflammatory bowel disease (IBD), or infections. They may also recommend dietary changes or tests for food intolerances.
2. Primary Care Physician (PCP): Your PCP can perform a comprehensive evaluation of your overall health, including blood tests to check for anemia, thyroid function, and other metabolic conditions. They can also assess your fatigue and recommend lifestyle changes or further testing.
3. Endocrinologist: If your PCP suspects a hormonal imbalance, particularly related to thyroid function, they may refer you to an endocrinologist. Thyroid disorders can significantly impact energy levels and metabolism.
4. Nutritionist/Dietitian: Consulting with a nutritionist can help you evaluate your diet and identify any potential deficiencies that may be contributing to your symptoms. They can provide personalized dietary recommendations to improve your overall health.
5. Sleep Specialist: If you suspect that your fatigue may be related to poor sleep quality, a sleep specialist can conduct a sleep study to evaluate for conditions such as sleep apnea, which can lead to excessive daytime fatigue.
Next Steps
1. Document Your Symptoms: Keep a detailed log of your symptoms, including when they occur, their severity, and any dietary habits. This information will be invaluable to your healthcare providers.
2. Schedule Appointments: Start with your primary care physician, who can help coordinate your care and refer you to specialists as needed.
3. Lifestyle Adjustments: Consider making some lifestyle changes, such as ensuring adequate hydration, maintaining a balanced diet, and managing stress, which can all contribute to improved health.
4. Follow-Up: After your initial consultations, ensure you follow up with your healthcare providers to discuss test results and any necessary treatment plans.
Conclusion
Your symptoms warrant a thorough evaluation by healthcare professionals. By consulting with the appropriate specialists, you can gain a clearer understanding of your health and work towards effective management of your symptoms. Remember, it's essential to advocate for your health and seek the support you need. Don't hesitate to reach out for help, and take care of yourself!
