Laser treatment for freckles can lead to the development of acne?
Hello, I would like to ask about my previous laser treatment.
I'm not sure what type of laser it was, but I had spot laser treatment that was done a month and a half ago.
I underwent a total of six sessions for melasma, with the last session being 14 days ago.
Recently, I've been experiencing breakouts on both of my cheekbones.
What should I do? Additionally, regarding the whitening cream, do I really need to apply it for as long as a year and a half? The nurse at the clinic where I had the laser treatment said I need to use it for that duration.
If I apply it for such a long time, could it lead to drug resistance or other issues for my skin?

Dr. Du Zhiqiang reply Dermatology
Hello, A-Hsuan: If you have developed acne in the areas treated with laser for pigmentation after multiple sessions, there are two possible reasons: (1) Excessive and frequent laser treatments may cause localized skin overheating or the dressing may not be breathable, leading to acne or folliculitis.
(2) After laser treatment, using skincare products or sunscreen that are not lightweight enough can also trigger acne, or applying too much whitening cream or spot treatment may result in acne as well.
It is advisable to consult the original physician who performed the laser treatment for interpretation or further evaluation.
The effectiveness of whitening creams is simply a matter of "effective or ineffective," and there is no issue of drug resistance.
Wishing you peace and health.
Sincerely, Dr.

More Info
Dealing with acne after laser treatment can be a frustrating experience, especially when you are unsure about the type of laser treatment you received. It is not uncommon for patients to experience breakouts following laser procedures, including those aimed at reducing pigmentation or improving skin texture. Here’s what you need to know about managing post-laser acne and the use of topical treatments.
Understanding Post-Laser Acne
1. Causes of Breakouts: After laser treatments, the skin undergoes a healing process. The laser can cause inflammation, which may lead to increased oil production and clogged pores, resulting in acne. Additionally, if the laser treatment was aimed at pigmentation, it may have temporarily disrupted the skin barrier, making it more susceptible to breakouts.
2. Timing of Breakouts: It’s important to note that breakouts can occur weeks after the procedure, as the skin continues to heal and regenerate. Since you mentioned that your last session was 14 days ago, it’s possible that the timing aligns with the natural healing process.
Managing Acne After Laser Treatment
1. Gentle Skincare Routine: Use a gentle, non-comedogenic cleanser to wash your face. Avoid harsh scrubs or exfoliants that can irritate the skin further. Look for products that are specifically formulated for sensitive or post-procedure skin.
2. Topical Treatments: Over-the-counter treatments containing salicylic acid or benzoyl peroxide can help manage acne. However, consult with your dermatologist before starting any new products, especially after laser treatment, to ensure they won’t irritate your skin.
3. Avoid Picking: It’s crucial to avoid picking or squeezing the acne, as this can lead to scarring or further inflammation.
4. Moisturization: Keeping the skin hydrated is essential. Use a lightweight, non-comedogenic moisturizer to help maintain the skin barrier.
5. Sun Protection: After laser treatments, your skin may be more sensitive to the sun. Use a broad-spectrum sunscreen with at least SPF 30 daily to protect your skin and prevent further pigmentation issues.
Regarding the Use of Whitening Creams
1. Duration of Use: The recommendation to use a whitening cream for up to 1.5 years is not uncommon, especially if the goal is to achieve significant lightening of hyperpigmentation. However, the duration can vary based on individual skin types and the specific product used.
2. Concerns About Resistance: While prolonged use of certain topical agents can lead to skin tolerance or reduced effectiveness, this is not universally applicable to all products. Ingredients like hydroquinone, for example, should be used with caution and under the supervision of a dermatologist, as they can cause side effects with long-term use.
3. Consultation with a Dermatologist: It’s advisable to discuss your concerns about the duration of use and potential side effects with your dermatologist. They can provide personalized recommendations based on your skin’s response to the treatment and suggest alternative options if necessary.
Conclusion
In summary, experiencing acne after laser treatment is a common issue that can be managed with a gentle skincare routine, appropriate topical treatments, and sun protection. Regarding the use of whitening creams, it’s essential to follow your dermatologist’s advice and communicate any concerns you may have. Regular follow-ups with your skincare provider will ensure that your treatment plan is effective and safe for your skin type. If the acne persists or worsens, consider scheduling an appointment with your dermatologist for further evaluation and tailored treatment options.
