Brown Semen: Causes and Seeking Solutions - Urology

The semen appears brownish, but it has already been examined?

Hello doctor, I have noticed that my semen has a distinctly brownish color with clumpy appearance, and it is almost entirely like this.
However, I do not experience any noticeable discomfort during ejaculation.
Recently, I went to the hospital for tests, including blood tests, ultrasound, and urinalysis, and the outpatient physician said the results were normal.
I am currently taking tranexamic acid, which I have been on for about 11 days, but there has not been any significant improvement in the appearance of my semen.
I am wondering if there could be other possible causes.
Thank you...

Hello, the color of your semen seems to be a result of old blood.
If there is no ongoing bleeding, after a few normal ejaculations, the residual blood should be expelled completely.
The more concerning issue is that blood is a relatively nutritious fluid; when it is inside the blood vessels, there is no infection risk.
However, once it flows outside the blood vessels, it can easily become contaminated and lead to infection.
Following an infection, new inflammation and bleeding can occur, creating a vicious cycle that becomes increasingly difficult to treat.
Therefore, I personally recommend administering hemostatic agents along with some antibiotics to prevent infection.
As for sexual activity, unless there is significant or bright red bleeding, there is no need to deliberately avoid it.

Brown semen, characterized by a coffee-colored appearance, can be concerning for many individuals. It is essential to understand the potential causes and seek appropriate solutions. The presence of brown or discolored semen often indicates the presence of blood, a condition known as hematospermia. While you have already undergone various tests, including blood work, ultrasound, and urinalysis, and have been prescribed tranexamic acid, it is crucial to explore further possibilities.

Possible Causes of Brown Semen
1. Infection: Infections of the prostate (prostatitis), seminal vesicles, or urethra can lead to blood in the semen. Symptoms may not always be present, but inflammation can cause discoloration.

2. Trauma or Injury: Any trauma to the pelvic area, whether from physical activity, medical procedures, or accidents, can result in bleeding that may manifest as brown semen.

3. Vascular Issues: Conditions affecting blood vessels, such as varicoceles or other vascular malformations, can lead to bleeding in the reproductive tract.

4. Benign Prostatic Hyperplasia (BPH): An enlarged prostate can cause bleeding, especially in older men, leading to changes in semen color.

5. Cancer: Although less common, prostate cancer or other malignancies in the reproductive system can cause hematospermia. This is particularly a concern if other symptoms are present, such as unexplained weight loss, pain, or changes in urinary habits.

6. Medications: Certain medications, including anticoagulants or those affecting blood clotting, can contribute to bleeding.

7. Other Medical Conditions: Conditions such as liver disease or clotting disorders can also lead to changes in semen color.

Next Steps for Diagnosis and Treatment
Since your initial tests have returned normal results, and you have been on tranexamic acid for 11 days without improvement, it may be time to consider further evaluation. Here are some recommendations:
1. Follow-Up with a Urologist: If you haven't already, consult a urologist who specializes in male reproductive health. They may recommend additional tests, such as a semen analysis, cystoscopy, or MRI, to investigate further.

2. Consider a Prostate Exam: A digital rectal exam (DRE) can help assess the prostate for any abnormalities that may not have been evident in imaging studies.

3. Review Medications: Discuss all medications and supplements you are taking with your doctor. Some may have side effects that could contribute to your symptoms.

4. Lifestyle and Dietary Changes: Sometimes, dietary factors or lifestyle choices can impact reproductive health. Ensure you are maintaining a healthy diet, staying hydrated, and avoiding substances that could irritate the urinary tract.

5. Monitor Symptoms: Keep a detailed record of any other symptoms you may experience, including changes in urination, pain, or any other unusual occurrences. This information can be valuable for your healthcare provider.

6. Seek a Second Opinion: If you feel your concerns are not being adequately addressed, seeking a second opinion from another healthcare provider can provide new insights or alternative treatment options.

Conclusion
While brown semen can be alarming, it is essential to approach the situation with a thorough understanding of potential causes and treatment options. Since your initial tests have not revealed any abnormalities, further investigation is warranted. Working closely with a urologist and being proactive about your health will help you find the underlying cause and appropriate treatment for your condition. Remember, open communication with your healthcare provider is key to addressing your concerns effectively.
