Abnormalities in semen and the epididymis?

Hello doctor, recently the semen I ejaculated was dark brown.
In fact, this happened about a month ago, but it gradually improved, so I didn't pay much attention to it.
However, after taking a hot spring bath today, it returned to the same condition as a month ago, with almost all of it being dark brown.
Additionally, I noticed that the right epididymis feels noticeably enlarged compared to the left when I check it with my hand (this has also been the case for about a month).
There is a slight discomfort when I squeeze it, but I don't feel any particular discomfort when I don't touch it.
Could this be chronic epididymitis? If so, is that the reason for the discoloration of the semen? Does it require surgical intervention for a complete cure, or can it be treated with oral medication? I have attached a link to a photo of the abnormal semen for your reference: http://i.imgur.com/Ivdpqsn.jpg.
Please feel free to check it.

It may be epididymitis; it is recommended to visit an outpatient clinic for examination.
The treatment is generally based on oral medications.

Dark brown semen can be concerning and may indicate underlying health issues. The color change in semen, particularly to a dark brown hue, can often be attributed to the presence of blood, which may result from various conditions affecting the male reproductive system. In your case, the accompanying symptoms of swelling and discomfort in the right epididymis raise the possibility of a condition such as chronic epididymitis.

Possible Causes of Dark Brown Semen:
1. Epididymitis: This is an inflammation of the epididymis, which can be caused by infections (bacterial or viral), sexually transmitted infections (STIs), or even non-infectious factors. The inflammation can lead to bleeding, which may cause the semen to appear dark brown.

2. Varicocele: This is an enlargement of the veins within the scrotum, which can lead to discomfort and changes in semen color due to altered blood flow and potential bleeding.

3. Prostatitis: Inflammation of the prostate gland can also lead to changes in semen color. Chronic prostatitis can cause pelvic pain and discomfort, and it may be associated with dark brown semen.

4. Injury or Trauma: Any trauma to the genital area can lead to bleeding, which may result in dark-colored semen.

5. Other Conditions: Less commonly, conditions such as tumors or cysts in the reproductive system can lead to changes in semen color.

Diagnosis and Treatment Options:
Given your symptoms, it is essential to seek medical evaluation from a healthcare provider. They may perform a physical examination, take a detailed medical history, and order diagnostic tests such as:
- Ultrasound: To assess the epididymis and surrounding structures for signs of inflammation, swelling, or other abnormalities.

- Urinalysis: To check for signs of infection or blood in the urine.

- Semen analysis: To evaluate the characteristics of the semen and identify any abnormalities.

Treatment:
1. Antibiotics: If an infection is diagnosed, antibiotics may be prescribed to treat the underlying cause of epididymitis or prostatitis.

2. Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help alleviate discomfort.

3. Rest and Support: Resting and using supportive underwear can help reduce discomfort associated with swelling.

4. Surgery: In cases where there is a significant structural issue (such as a large varicocele or abscess), surgical intervention may be necessary. However, this is typically considered only after conservative treatments have failed.

5. Follow-Up: Regular follow-up with your healthcare provider is crucial to monitor your condition and adjust treatment as necessary.

Conclusion:
In summary, dark brown semen accompanied by swelling and discomfort in the epididymis is a sign that warrants medical attention. While chronic epididymitis is a possibility, other conditions could also be responsible for your symptoms. It is essential to consult a healthcare professional for a proper diagnosis and treatment plan tailored to your specific situation. Early intervention can help prevent complications and improve your overall reproductive health.

Your symptoms may be indicative of hematospermia. Medically, the causes of hematospermia are mostly benign, and the majority are self-limiting, meaning they may resolve without treatment. However, if you are concerned, it is still advisable to consult a urologist.
