What is wrong with me, doctor?

Doctor: This condition has been present for about 5 years.
Recently, there have been frequent episodes of sharp pain below the right rib cage, which makes it difficult to breathe (inhaling causes pain).
Occasionally, there is also pain on the left side.
What could be the cause? If I need to see a doctor, which specialty would be best? Thank you, doctor.

Right rib pain can be a perplexing symptom, especially when it has persisted for an extended period, as in your case of five years. The pain you describe, particularly the sharp, stabbing sensation beneath the right rib cage that worsens with deep breaths, can be attributed to various underlying conditions. It is essential to consider both gastrointestinal and musculoskeletal causes, as well as potential issues related to the liver or gallbladder.

Given the location of your pain, one possibility is that it could be related to the liver or gallbladder. Conditions such as gallstones, cholecystitis (inflammation of the gallbladder), or liver issues can manifest as pain in the right upper quadrant. However, since you mentioned that you have had previous evaluations, including endoscopy and ultrasound, which did not reveal significant abnormalities, it may be less likely that these organs are the primary source of your discomfort.

Another potential cause could be related to the musculoskeletal system. Costochondritis, which is inflammation of the cartilage connecting the ribs to the sternum, can cause sharp pain that may be exacerbated by deep breathing or certain movements. Similarly, muscle strain or injury in the intercostal muscles (the muscles between the ribs) could lead to the symptoms you are experiencing. If you have been engaging in activities that involve heavy lifting, twisting, or repetitive motions, this could contribute to your pain.

Additionally, conditions such as gastroesophageal reflux disease (GERD) or peptic ulcers can sometimes present with referred pain in the upper abdomen or chest, which may be perceived as rib pain. If you have a history of gastrointestinal issues, it may be worth discussing these possibilities with your healthcare provider.

Considering the duration and nature of your symptoms, it is advisable to consult a gastroenterologist, especially if you have not done so already. A gastroenterologist specializes in disorders of the digestive system and can provide a more focused evaluation of your symptoms. They may recommend further diagnostic tests, such as a CT scan or MRI, to assess the abdominal organs more thoroughly, especially if there is a suspicion of structural abnormalities that were not identified in previous imaging studies.

In summary, while your right rib pain could stem from various causes, including gastrointestinal, musculoskeletal, or even referred pain from other organs, a thorough evaluation by a gastroenterologist is warranted. They can help determine the underlying cause of your symptoms and recommend appropriate treatment options. It is crucial to address this issue, especially since the pain has been persistent and is affecting your quality of life. Don't hesitate to seek medical attention to ensure a comprehensive assessment and management of your condition.
