Pain below the right rib cage?
Hello, doctor.
My son is currently a sophomore and just turned 19.
In the past month, he has experienced two episodes of severe pain under his right rib cage.
Coincidentally, both episodes occurred at night.
In urgent situations, we administered pain relief medication, even though we know it may not be appropriate.
We are concerned that this could be related to liver or gallbladder issues, or possibly something else, and we are unsure which specialty to consult for medical care.
We would greatly appreciate your guidance.
Thank you!
Fanny, 40~49 year old female. Ask Date: 2012/10/12
Dr. Chen Shidian reply Gastroenterology and Hepatology
Hello Fanny, if it is inconvenient for you to seek medical attention, you can apply local heat to alleviate the pain.
You may also use appropriate pain relievers and antacids.
You can visit the gastroenterology outpatient clinic for an ultrasound or endoscopy to confirm the medication.
Changhua Hospital cares about your health.

Right upper quadrant (RUQ) pain can be a concerning symptom, especially when it occurs suddenly and severely, as you described in your son's case. Understanding the potential causes and when to seek medical help is crucial for proper diagnosis and treatment.
Common Causes of Right Upper Quadrant Pain
1. Gallbladder Issues: One of the most common causes of RUQ pain is related to the gallbladder, particularly gallstones or cholecystitis (inflammation of the gallbladder). Patients often experience sharp pain after eating fatty meals, accompanied by nausea or vomiting.
2. Liver Conditions: Liver diseases, such as hepatitis or liver abscesses, can also cause RUQ pain. The liver is located in the upper right abdomen, and any inflammation or enlargement can lead to discomfort.
3. Kidney Problems: Conditions affecting the right kidney, such as kidney stones or infections, can present as pain in the RUQ. This pain may radiate to the back or lower abdomen.
4. Gastrointestinal Issues: Conditions like peptic ulcers, pancreatitis, or even constipation can lead to RUQ pain. The pain may be accompanied by other gastrointestinal symptoms such as bloating, gas, or changes in bowel habits.
5. Musculoskeletal Pain: Sometimes, the pain may not be related to internal organs but rather to muscle strain or rib injuries, especially if there has been recent physical activity or trauma.
6. Other Causes: Other potential causes include pneumonia (especially if the lower lobe of the right lung is involved), herpes zoster (shingles), or even referred pain from other areas of the body.
When to Seek Medical Help
Given that your son has experienced severe pain twice in a month, it is advisable to seek medical attention, especially if:
- The pain is severe and sudden.
- It is accompanied by other symptoms such as fever, jaundice (yellowing of the skin or eyes), persistent vomiting, or changes in bowel habits.
- The pain does not improve with over-the-counter pain relief.
- There is a history of gallbladder disease or liver issues in the family.
Recommended Steps
1. Consult a Doctor: It would be best to consult a healthcare provider, preferably a gastroenterologist or a primary care physician. They can perform a thorough evaluation, including a physical examination and possibly imaging studies like an ultrasound or CT scan to assess the liver, gallbladder, and other abdominal organs.
2. Avoid Self-Medication: While it may be tempting to use over-the-counter pain relievers, it's crucial to avoid self-medicating without understanding the underlying cause of the pain. Some medications can mask symptoms and delay diagnosis.
3. Keep a Symptom Diary: Documenting when the pain occurs, its intensity, duration, and any accompanying symptoms can be helpful for the healthcare provider in making a diagnosis.
4. Lifestyle Considerations: Encourage your son to maintain a healthy diet and avoid fatty foods, which can exacerbate gallbladder issues. Staying hydrated and managing stress can also be beneficial.
In conclusion, while RUQ pain can stem from various causes, it is essential to take it seriously, especially with recurrent episodes. Seeking medical advice promptly will help ensure that any serious conditions are ruled out and that appropriate treatment is initiated.
